摘要:
The health care delivery system is undergoing evolutionary changes that are also affecting Clinical Engineering. The integration of engineering and the life sciences created an industry whose “product” must be quality patient care. The utilization of technologies in the clinical environment is perpertually growing, and has created a need for professional technical management. The present changing environment requires Clinical Engineers to become effective leaders and efficient managers.The efficient consumption of an organization's resources is dependent on its managers' abilities to assess and optimize their operations under dynamic conditions. This paper describes some means for monitoring the clinical engineering department “output” and for measuring and reporting the relative changes in output, thus enhancing progress toward achievement of established goals. The tools and techniques offered here are not an end in themselves, but are rather a part of the process of maximizing productivity with a commitment to program output quality.

摘要:
This paper describes the Biomedical Services of Northern Montana Hospital, a separate corporation under the parent corporation of Northern Montana Health Care Inc. Located in Havre, Montana, a rural area of north central Montana, the hospital is a 120-bed, non-profit organization. The two BMETs work as part of the Engineering Department and service almost every piece of equipment in the building (approximately 1100 items), as well as a telephone system for another facility. This paper shows that a non-urban hospital, located far from any service company outlet, can be self-supporting as far as equipment service and maintenance are concerned.

摘要:
Software advancements have allowed significant progress in the development of computer expert systems. A case study was done with the goal of producing a prototype system to aid in the diagnosis of cardiovascular diseases using such advanced software techniques. A description of the system, named CARDIOLOGIST, is presented in this paper, along with a status report and a brief overview of the software tools used.

A Motorola 68020 microprocessor-based system has been developed for the analysis of the ST-segment in ambulatory ECG recordings. The system is compatible with FM cassette, as well as amplitude modulated (direct) reel-to-reel and cassette recording units. The software algorithm, written in Pascal and assembly language, permits R-wave detection and pattern recognition, using both cross-correlation and feature-extraction methods. Family clustering of all QRS waveforms is accomplished by the computer before operator intervention is required for verification of the sinus beats to be selected for ST-segment processing. Electronic markers are then placed on the isoelectric line, J-point, and ST-segment for quantitative determination of ST-segment elevation or depression. All episodes of ST-segment deviation greater than 1.0 mm are verified by an operator interactively on a CRT display. The analytic method is artifact resistant and reproducible for ST-segment changes and for the duration of each event with correlation coefficients greater than 0.94. The technique has been shown to have a sensitivity of 100 percent for the detection of ST-segment changes greater than 1.0 mm.

摘要:
Extensive monitoring, long hospital stays, and high costs are often associated with head injury patients. This paper discusses the different types and causes of head trauma. Methods for preventing these injuries are of utmost importance. An overview of the development and mechanistic evaluation of the standards for helmets is presented, including vehicular, athletic, and industrial helmets currently in use. A comparison of the criteria used in the design of standards leads to a better understanding of potential injury factors and methods for injury prevention.
