摘要:
Finances have become the dominant concern of hospital administrators and department heads. Clinical Engineering (CE) can make significant contributions to the financial health of a hospital by increasing CE departmental productivity and by improving the utilization of resources in clinical departments. Several measures of productivity and cost-effectiveness have been applied to the Biomedical Engineering Department of the University Medical Center. The Department provides a wide range of technical services that are integrated into the clinical and administrative activities of the hospital. The Department has accumulated data regarding the financial benefits provided to the hospital, and the data reveal significant savings which show that CE can be viewed as a cost-effective investment. The greatest savings occur in capital equipment acquisition (selection and installation) and maintenance, and result from CE involvement in clinical activities and administrative decision making.

This paper describes the Biomedical Services Division of Heritage Health Management Corporation, a subsidiary of the SwedishAmerican Corporation. Located at SwedishAmerican Hospital in Rockford, IL, the Division operates as an independent agency servicing 20 facilities. The staff of ten full-time and two part-time employees is responsible for over 5000 pieces of equipment. In addition to in-house maintenance, field service is accomplished with the aid of a fully-equipped van.

A system that was developed to enable paraplegics and quadriplegics to stand and walk is described in this paper. This system is a combination of the Louisiana State University (LSU) reciprocating gait orthosis (RGO) and functional electrical stimulation (FES). The system uses closed-loop control of the knee. Electrical stimulation of the hamstring muscles, quadriceps muscles, and gluteus maximus muscles was accomplished under computer control for standing, sitting, and walking in subjects in the RGO. The braces offer the advantages of postural stability and protection from injury if a fall were to occur. Electrical stimulation increases the efficiency and speed of movement, making a more practical system than use of the braces alone. Electrically conductive clothing can be used to stimulate the muscle transcutaneously and, thereby, eliminate the need for numerous wires. Consequently, a system can now be fabricated that is much easier to use, more cosmetic, and requires less maintenance than previous walking systems.

There is a need for a simple and accurate measurement technique to assess skin blood flow for amputation surgery or tissue perfusion for evaluation of peripheral vascular disease. The authors describe a device that can quickly and easily provide multiple measurements related to surface tissue perfusion. The device directs an infrared light source into the tissue and detects blood pulsations under the skin as changes in reflected light. Gradually applying a force to the light source and against the skin causes the pulsations to decrease and disappear as the external force overcomes tissue blood pressure. Measurements are made of initial pulse height, force required to extinguish pulses, and subsequent hyperemic pulse amplitude. The device was tested on ten normal subjects and three patients with known vascular disease. Results indicate that the measured hyperemic response provides a measure of the adequacy of tissue circulation (.001 level of significance) when compared to initial pulse height. Measurements of force for pulse extinction were inconclusive; suggested modifications are included.

This paper describes some of the many administrative and logistic problems encountered while developing and implementing a project to provide medical equipment to third-world countries. Specific areas addressed are the importantance of a survey of the facilities and needs, how to determine the level of involvement that can be assumed, the impact on the recipient facility from that involvement, how to locate personnel and equipment sources, and criteria for developing and monitoring such a project.

This paper presents an overview of the biomedical engineering programs located in the states of Arkansas, Kansas, Missouri, Oklahoma, and Texas. Two-year, four-year, and graduate programs are described, including contact information, course requirements, and prerequisites. As Part II of the Journal's continuing series on education, the intent of the paper is not to evaluate each program, but to illustrate the breadth of biomedical and related engineering education programs available today and to serve as a useful reference to such programs.
