摘要:
A system for ambulatory monitoring ofsystolic time intervalshas been developed. This objective was accomplished by adaptation of a standard two-channel, long-term electrocardiogram monitoring unit (Holter Monitor) in which the second channel was modified to record a pulse tracing, while the first channel recorded a bipolar V5ECG. The pulse tracing used was a derived ear densitogram singal.From the two-channel printout, measurements ofpre-ejection periodandleft ventricular ejection timecould be computed, as long as pulse transmission time was prerecorded.Systolic time interval measurements were recorded on four subjects for several hours. The values of systolic time intervals obtained and the changes in systolic time intervals on each subject were consistent with those reported in the literature.

It is proposed that there is something basically wrong with our system for developing national safety standards. The present system seems to waste a large proportion of the collective wisdom available within the expert technical committees. An annual technical meeting where these issues could be discussed openly by technical committee members and others is proposed. There is a strong belief within the hospital community that the technical committees can be too easily swayed by a minority of extremists who see the best solution as proposing safeguards against every conceivable hazard. Some of the requirements in the present codes impress the hospital staff as ridiculous, such as having to wear conductive shoe covers in the operating room even when the hospital has adopted a policy that only nonflammable gases be used. Unless the logical basis for these apparently irrational requirements can be made clear, a cynical attitude toward safety codes cannot be avoided.

A useful circuit is described which utilizes an LM3905N integrated circuit timer to control the operation of a commercial pulse generator or stimulator. The circuit allows generation of precisely timed and repeatable stimulus pulse trains. The specific modification of a Grass Model SD9 stimulator is described. The pulse train interval is variable from approximately 0.5 to 10.5 seconds, but other timing ranges are possible with a simple circuity change. This modification has a low parts cost, is easy to construct and attach to a commercial stimulator, and does not alter the normal operation of the stimulator. Construction of this stimulator modification could be considered as an alternative to the purchase of an additional commercial unit. The calibration procedure and the operating instructions are described. The LM3905N integrated circuit is simple to use and has many possible timing applications in clinical engineering.

An effective electrical safety program requires a cooperative and conscientious hospital staff, a functional Electrical Safety Committee, and a wellorganized Clinical Engineering Department. In this triumvirate, the Electrical Safety Committee provides the requisite administrative framework, with the safety policies and standards serving as the program's charter. The composition of these policies is a primary determinant of the program's success or failure.These policies and standards must be configured to suit the circumstances of each hospital. They must be formulated so that the operation of electrical/electronic apparatus presents minimal patient hazard without limiting the type or performance of such devices. In their generation, consideration must be given not only to instrument design but also to other factors such as maintenance, purchasing, and operator training.This paper will discuss our institution's experience in generating electrical safety policies and standards. In addition, it will provide some insight into the utility of these standards in an electrical safety program.

摘要:
This article defines a “placebo effect” and identifies some of its parameters in pain control and in other areas of medicine. it proposes a new model of the placebo effect and advances the hypothesis that biomedical instruments used in biofeedback studies, like drugs, can acquire and generate placebo effects. Such placebo effects can complicate the interpretation of specific experimental treatments in human clinical research in which biomedical instruments are used.
