摘要:
This paper presents definitions of the terms bioengineering, biomedical engineering and clinical engineering. These definitions lead to the conclusion that clinical engineers must be individuals with at least a four-year bachelor's degree in an engineering specialty who are also well versed in the design, modification and testing of medical instrumentation. Educational programs for clinical engineers in the 1990s must be based upon clear definitions of these professionals' roles. Clinical engineering education should include direct professional experience obtained through internship programs similar to the program described here.

摘要:
A customized instrumentation stand is described for use during transport of cardiothoracic or major surgery patients between the OR and a critical care area. The stand attaches to a patient bed and is equipped with a pulse oximeter, ECG/BP monitor, defibrillator/monitor and support module, oxygen tank, supplies, and medications. The design allows maximum visibility and accessibility, easy movement, and quick attachment to and disconnection from a bed. A similar apparatus may benefit surgical programs facing comparable transport needs.

Eye-level blood pressure was indirectly measured in human superficial temporal arteries with a newly designed sphygmomanometer using the volume oscillometric principle. This device comprises a reflected, infrared, photoelectric plethysmograph (RIPP) for detecting arterial volume variations and an artery compression cuff. Systolic and mean arterial pressures were determined from the cuff pressure corresponding respectively to the appearance and maximum point of the arterial volume variations. Diastolic pressure was calculated from these pressure values using a conventional equation. The pressures were corrected with respect to the hydrostatic pressure difference between the heart and eye, and compared with the data simultaneously obtained by (1) auscultation method in the brachial arteries; (2) volume compensation; and (3) a volume oscillometric method in the finger arteries. The eye-level pressure and its change were recorded during rest and exercises such as bicycle ergometry, weight lifting, Master's two-step test, and acceleration loading by a human centrifuge.

This paper presents the design of a programmable cardiac stimulator which is incorporated into a broader system of acquisition, processing and display of electrocardiographic signals. The stimulator was devised for use in arrhythmia surgery and electrophysiologic studies; it is implemented by means of two microprocessors, A and B, connected by an RS232 serial communication. Microprocessor A manages the user interface: console display, control function keys, data input and transmission of stimulation parameters. Microprocessor B processes the information received and implements the appropriate stimulation. Six available modes of stimulation are described as well as the parameters involved and the range of values that they cover. The auxiliary circuitry of microprocessor B is described in detail. It consists of four blocks: selection of the stimulation channel, conformation of the stimulation pulse, amplifier-differentiator sensing, and the subsystem for the study of the rheobase curve. Two general flowcharts and a description of the major routines are provided.

A materials management system capable of inventory control, accounting and the automatic recording of supplies for a clinical department has been developed for the George Washington University Hospital Department of Anesthesia. This system combines a microprocessor-based computer for data storage and a hand-held bar code reader to record the bar code scan of each item in the inventory. A relational software program with easy-to-use menus and help keys was written. Bar code information stored for each item includes item number, quantity, date and time of issue. Accumulated bar code scans are loaded into the computer by use of a serial port and then used to update current inventory in the computer. Comparison between current inventory and reorder levels by the computer will initiate automatic printing of appropriate purchase orders. Reorder levels are adjusted regularly, by comparing previous year or month usage to current needs; items already on order, items on back order and delivery lag time are also taken into account.
