DRGs—these letters, spoken as frequently as they now are, may sound familiar and perhaps even sinister. They have rapidly become the driving force of the health care system. Medicare is the largest purchaser of health care and has engineered a financing program based uponDRGs.Since other insurers are joining the program, the health care industry, along with its suppliers and clients, will never again be treated as it was in the past. As participants in the system, our work and our careers will also be changed. Yet, as with any change, there will be opportunities to acquire knowledge, to grow professionally, and to contribute to society. If hospitals are to provide quality care in a cost-effective and efficient manner, it will be both because of and in spite of technology. The appropriate selection, use, and maintenance of technology is an intrinsic part of the system. The clinical engineer's role will be to contribute technical and managerial expertise in support of the institution—in other words, in the management of technology.

This review considers the role ofartificial intelligence(AI) in medical informatics. It begins by describing artificial intelligence and then places it in the context of medical computing. Knowledge processing is emphasized. Following a brief review of the state-of-the-art in medical expert systems, a final section considers howartificial intelligence in medicine(AIM) can provide a focus for medical informatics that emphasizes the acquisition, evaluation, and utilization of medical knowledge.

摘要:
Servicing of microprocessor-based and digital patient care instrumentation presents a special challenge to the in-house clinical engineering program. The complexity of this instrumentation, the cost of test equipment and support materials, and continuing education needs are pressing concerns. Servicing methods available to clinical engineering include the use of manufacturer diagnostics and board or subassembly swapping. Test equipment, such as digital oscilloscopes, logic analyzers, signature analyzers, and logic probes, can also be used during repair, even to the component level. In some instances, clinical engineering may choose alternative service arrangements, such as fee-for-service, service contracts, or maintenance insurance. Clinical engineering must objectively evaluate its resources and the level of technical expertise within the department; a realistic maintenance program for microprocessor-based and digital patient care equipment can thus be developed and can result in significant cost savings.

摘要:
On-line analysis of cardiovascular signals is generally a difficult task; however, cardiovascular signals often have certain characteristics that simplify this task. These characteristics are summarized in the first part of this paper. The main features of a microprocessor-based system for the on-line analysis of cardiovascular signals are outlined and results presented. The system software is easy to modify to fit the requirements of several types of biological experiments or clinical measurements. A prototype was built and employed for studying the effects induced in cardiovascular systems by drug administration.

摘要:
The design and construction of an electronic adapter, which allows the Hewlett Packard (Waltham, MA) Model 1290A pressure transducers to be used with the Spacelabs, Inc. (Chatsworth, CA, formerly Tektronix and Vitatek) Model 414 single-channel portable patient monitors, is described in this paper. The adapter has no operator controls and requires no electrical modification to the Spacelabs monitor other than connections to the monitor power supply. The adapter can also be unplugged from the patient monitor to allow use of the original Spacelabs-compatible pressure transducers.

A photoelastic coating stress analysis technique was used to visualize surface stress distributions in bone plates and in a melamine formaldehyde bone model subjected to bending loads. This technique allows rapid identification of stress concentrations by the observation of photoelastic fringe patterns. This photoelastic coating technique could be useful in evaluating the potential for fatigue failure in bone plates.

摘要:
A microcomputer-based device for evaluating alternate pronation and supination of forearms (diadochokinesia test) has been implemented. Forearm movements are converted into digital signals through an A/D converter, and computations are performed to determine the rapidity and regularity. As test results, the tracings of the movements and the computed measurements are produced on a small plotter. The device has been made compact enough to be easily carried around in a clinical ward. The test may be performed at the bedside and the findings obtained immediately. The device has been applied to cases with cerebrovascular disease, and its usefulness in clinical practice has been shown.
