Subacute focal encephalitis of Rasmussen involves cortical inflammatory infiltration which may on occasion be bilateral. The presence of anti-Glu R3 antibodies in some patients suggests that an autoimmune process may be involved. Although a small number of patients respond transiently to immunoglobulins and the condition of some is partially improved by steroids, only hemispherectomy seems to produce prolonged relief of seizures. Subpial cortical transection may be useful if hemiplegia has not developed.

A recent videometric analysis of 56 brief syncopes showed myoclonic activity in 90%, together with head turns, upward gaze, oral automatisms, righting movements and visual and auditory hallucinations in 60–80%. Characteristic precipitation factors, premonition features such as nausea, sweating and palpitation, brief duration (<25 sec), absence of paroxysmal electroencephalogram activity and no evidence f or disorientation, exhaustion or sleep after the attack, are useful discriminating factors.

In the past year, magnetic resonance has assumed an increasingly important role in the study of patients with epilepsy that now extends beyond its demonstration of neuroanatomy and neuropatholgy into the fields of neurochemistry, neurometabolism and neurophysiology. For proper use of this information, epileptologists, neurologists and neuroradiologists need to understand the limitations as well as the strengths of this technology. Magnetic resonance literacy will increasingly become an essential part of all of these disciplines.

Neuroimaging techniques have had a dramatic impact on the evaluation and treatment of patients with epilepsy. In order to take full advantage of their potential, it is important to place them in clinical and electrophysiological context and to understand their technical limitations. Positron emission tomography with18F-2-deoxyglucose and single photon emission computed tomography can provide valuable data for presurgical localization of epileptogenic zones. Interictal cerebral blood flow studies, however, using either positron emission tomography or simple photon emission computed tomography are unreliable. Positron emission tomography cerebral blood flow activation studies, on the other hand, are becoming very useful for presurgical cognitive mapping and may be able to replace the intracarotid amytal test for language and memory lateralization. There are a number of receptor ligands available for both positron emission tomography and simple photon emission computed tomography studies, including benzodiazepine, opiate, and cholinergic tracers. Increased mu opiate, decreased benzodiazepine, and increased monoamine oxidase B receptor binding have been reported.

The overall risk of premature death in patients with epilepsy has recently been quantified and is two to three times that of the general population. Most of the increased risk is directly related to the cause of the epilepsy itself. A small number of deaths in the general population of patients with epilepsy are epilepsy-related deaths, which includes those deaths caused by a seizure or accidents during or immediately after a seizure, as well as sudden unexpected death, for which no cause of death can be identified. Sudden unexpected deaths are more common in patients with intractable epilepsy. Recent studies suggest that improved seizure control, advice and help to avoid the hazards that occur during a seizure, as well as increased supervision of institutionalized patients, may help reduce the risks of epilepsy-related death occurring.

Since the time of Sommer's review of hippocampal pathology and Gower's tenet that 'seizures beget seizures' there has been interest in changes in brain structure and function that may be associated with epilepsy. From recent experimental studies and clinical observations, there is increasing evidence that epilepsy is not only associated with structural and functional alterations in the nervous system, but also that seizures induce a diverse variety of molecular, cellular, and functional alterations in the brain.

The physician's choice of antiepileptic drug for a particular patient should be based on firm evidence. The current body of evidence generated from randomized, controlled monotherapy trials is discussed. Attention is focused on four recently published randomized trials in which monotherapy with one drug is compared with monotherapy with another. The limitations of our current knowledge are discussed and some suggestions for future research are made.
