AbstractA retrospective study of 1,855 fine‐needle aspirations of the lymph node (LN) from 1,484 Chinese patients in Queen Mary Hospital over a period of 11.5 yr (January 1, 1976, to June 30, 1987) was reviewed and analyzed. Positive cytologic results diagnosed malignancies in 49.3%. Sensitivity and specificity of cytology reached 95% and 96.5%, respectively. The order of frequency of the metastatic tumors according to primary sites was analyzed. The most common metastatic carcinomas in various groups of LNs were nasopharyngeal carcinoma in the cervical LN. breast carcinoma in the axillary LN, and cervical carcinoma in both the groin and pelvic LNs. AbstractThis study used CA125, carcinoembryonic antigen (CEA), and alpha‐fetoprotein (AFP) to classify ovarian cysts by measuring the levels of the three antigens; this information was useful when fluid obtained through laparoscopic puncture of ovarian cysts was submitted for cytologic examination from patients for whom tissue was unavailable for classification. We studied 136 consecutive cyst fluids (108 benign, 28 malignant) and correlated the findings with the tissue diagnosis. All three antigens were very low (CEA,<0.5 ng/ml; CA125, 55‐2,143 μ/ml; and AFP,<4.8 ng/ml) in follicular and lutein cysts. Markedly elevated CA125 (296‐1,950,000 μ/ml) and low CEA (0.5‐220 ng/ml) and AFP (600 ng/ml) and a normal CA125. Only one patient, with a malignant teratoma, had an elevated AFP. The adjunctive use of CEA and CA125 is recommended for the classification of

AbstractPneumocystis cariniipneumonia(PCP) is the major pulmonary complication in patients with the acquired immunodeficiency syndrome. While fiberoptic bronchoscopy with bronchoalveolar lavage (BAL) and transbronchial biopsy (TBBX) is regarded as the procedure of choice to identify PCP, these techniques, particularly TBBX, pose potential risks to the already compromised patient. To reduce the duration of bronchoscopy and, hence, lessen the chance for complications, we describe a rapid technique to identify PCP in BAL fluid by cytocentrifugation and hematoxylin‐eosin staining. By this method, PCP can be easily diagnosed within 10 min. The sensitivity of this rapid diagnostic procedure is 95.7%, and the specificity is 100

AbstractTo date, we have studied 89 palpable lesions of the scrotum, testicle, and epididymis using fine‐needle aspiration cytology (FNAC). Cystic lesions (48.3%) and inflammatory pathology (25.8%) were the most frequent findings. Tumors accounted for 11.2% of results, with a slight predominance of the malignant varieties. In this article, we describe the cytopathology of the main entities and discuss the principal problems of differential diagnosis, especially among inflammatory processes, seminomas, and embryonal carcinomas. The cytopathological pattern of the neoplasias is highly characteristic, and this permits their diagnosis with great precision. FNAC is essentially non‐traumatic and easy to carry out, but it requires considerable practice in its execution and in the interpretation of the aspirates. We believe FNAC to be the technique of choice for the study of the pathology of the scrotal content, and we think that it should be employed on the patient's very first visit. AbstractThe classic cytologic criteria, koilocytotic atypia and dyskera‐tocytosis, fail to identify most cases with colposcopic and histologic evidence of cervical condylomata. The purpose of this study was to identify a novel cytologic diagnostic criterion, spindled nuclei, to predict the presence of human papillomavirus (HPV) infection of the cervical epithelium. Review of the hospital records of 100 consecutive cases with spindled nuclei on Papanicolaou smear identifed 36 patients in whom a colposcopic examination and/or cervical biopsy had been performed between January 1, 1988, and March 31, 1989. Ninety‐seven percent of these 36 cases were positive by colposcopy and 89% of the cases were positive by cervical biopsy for changes of condyloma or intraepithelial neoplasia. HPV DNA hybridization in situ was positive in 16 of 36 patients, and the probe for types 31/33/35 most often stained histologic sections showing condylomatous change without concurrent dysplasia. AbstractFine‐needle aspiration biopsies from five patients with endo‐dermal sinus tumors (ESTs) were reviewed, and the j'indings were correlated with histologic and ultrastructural (appearances. In the aspiration smears, two types of tumor cells were seen, forming clusters of variable sizes. Type A cells had distinct cell borders, and their cytoplasm contained onry occa‐ sional vacuoles. Type B cells had ill‐defined cell borders and formed syncytial clusters; their cytoplasm was characterized by large numbers of rounded vacuoles. The background con‐ tained patches of mucoid material and macrophages with foamy cytoplasm. Eosinophilic hyaline cytoplasmic bodies and irregular deposits of intercellular basement membrane‐like material were recognized easily in aspiration smears. These features correlated well with histologic and ultrastructural appearances. AbstractWe report the cytologic findings of small intestinal brushing material showing cryptosporidiosis in two patients with acquired immunodeficiency syndrome (AIDS). Cryptosporidia measuring approximately 2–4 μ were aligned along the surface of the glandular cells, although many freely dispersed organisms were also present. The differential diagnoses of other structures that could be confused with cryptosporidia are also presented in this article. AbstractPeculiar ring‐like structures identifed as Liesegang rings (LRs) were found in renal cyst fluid from three patients with benign renal cysts. They ranged in size from 5 to 820 μ. Most had a double‐layer outer wall with equally spaced radial cross‐striations and an amorphous central nidus. Special stains were performed in one case, and the results are discussed. Reports of LRs in cystic or inflamed tissues have recently appeared in the literature. Some LRs have been mistaken for eggs or mature components of the giant kidney worm,Dioctophyma renale.We propose that cytologic assessment of renal cyst fluid in conjunction with histologic examination decreases the likelihood of misdiagnosis

AbstractPseudomyxoma peritoneiis the clinical term for the diffuse deposition of mucus within the peritoneal cavity secondary to a mucinous tumor of the ovary or appendix. This gelatinous ascites, or ‘jelly‐belly’ may result in death from loss of intestinal function and intestinal obstruction caused by peritoneal implants rather than visceral invasion.
