AbstractThe authors systematically analyze a series of 38 patients having a total of 40 vitreous fluid cytology determinations. Nine of the 38 patients (23.7%) have cytologic results diagnostic for, or at least highly suggestive of, a specific disease process. These include two cases of bacterial endophthalmitis; two cases of fungal endophthalmitis; one case of vitreal amyloidosis; and four cases called positive for intraocular lymphoma. There are no vitreous fluid cytology determinations falsely positive or falsely negative for malignancy in this series. Intraocular lymphoma and the increasingly important role of vitreous fluid cytology examination in its diagnosis are discussed. The authors conclude that a strong commitment to quality control concepts, including detailed clinical follow‐up, is essential in order to develop clinician trust and help achieve optimal patient care benefit from this valuable diagnostic modalit

AbstractAn ultrastructural study was performed on 104 sequential fluids in which more than eight malignant cells per ten highpower fields were found by routine light microscopy. The study included fluids associated with mesotheliomas, melanomas, lymphomas, squamous‐cell carcinomas, small‐cell anaplastic (oat‐cell) carcinomas, and adenocarcinomas. Electron microscopic examination reliably separated lymphoid from epithelial malignancies and benign from reactive and malignant mesothelial cell proliferations. It also suggested or identified a primary site for the adenocarcinomas. Ultrastructural examination of fluids can be a valuable adjunct to routine light microscopy of cytology specimens. AbstractFrom 1,488 patients, satisfactory sputum was available for cytologic diagnosis. Overall diagnoses were correct in 85.4% of patients, false negative in 193 patients (13.0%), and false positive in 24 patients (1.6%). In patients with a malignant lung process, cytolagic diagnoses were correct in 228 patients (54.2%) and false negative in 193 patients (45.8%). In patients with primary lung cancer, the proportion of correct positive diagnoses increased from 0.47 to 0.87 with one to five sputum specimens examined. In patients with metastatic disease, the figures were 0.35 with one specimen examined and 0.38 with two and more sputum specimens. Cytologic typing accuracy was 67% for large‐cell carcinomas, 73% for adenocarcinomas, 91% for small‐cell lung cancers, and 98% for squamous‐cell carcinomas. AbstractHerpes simplex virus (HSV) infection of the respiratory tract requires rapid specific diagnosis to avoid late complications and maximize the efficacy of available drug therapy. We report a method of accomplishing this that was tested in 33 cytologic specimens derived from sputum, washings, or brushings from 25 debilitated elderly males suffering from a variety of underlying neoplastic and nonneoplastic chronic diseases. All specimens had shown either single cells (54%), multinucleated groups (8%), or both (38%); they displayed the ground‐glass appearance (86%), discrete nuclear inclusions (4%), or both (10%), as appreciated by the Papanicolaou stain. The specimens were processed by the peroxidase‐antiperoxidase technique utilizing anti‐HSV‐1 antibody and 3,3′‐diaminobenzidine (DAB) as the chromogen. In six cases, aminoethylcarbazol (AEC) was used for comparison. Twenty‐nine of the 33 specimens (88%) stained positively for HSV‐1 as did control sections of herpetic encephalitis and esophagitis; there were no false positives with appropriate negative controls. All 12 bronchoscopic specimens revealed virocytes with HSV immunopositivity; in contrast, 17 of 21 (80%) sputum specimens were positive for HSV. The strongest positivity was noted in bronchial brushings and washings whereas the only four negative smears were from sputum specimens. The DAB immunostain was coarser and stronger at the periphery of the cytoplasm, and the hematoxylin counterstain permitted a clear identification of nuclear viral changes. With AEC, the immunostain was more vivid and evenly distributed, but counterstaining was impaired due to the greater solubility of the chromogen. AbstractThirty‐five pulmonary carcinomas were studied retrospectively with monoclonal antibody (MCA) 44‐3A6 raised against a human adenocarcinoma cell line. The antibody was applied to cytologic smears of bronchial brushings originally stained with the Papanicolau method, and to conventional tissue sections. Ten of 12 adenocarcinomas (ADC) immunostained strongly in sections and smears, as did five of seven large‐cell „undifferentiated”︁ carcinomas (LCUC). Eight neuroendocrine carcinomas (NEC) and eight squamous‐cell carcinomas (SCC) were negative, except for rare weakly positive foci. We conclude that MCA 44‐3A6 can be effectively applied on cytologic smears, and that it could be valuable in the precise classification of pulmonary carcinomas. The immunoreactivity of the ADC and SCC was predictable. Positive immunostaining in some LCUC confirms that these constitute a heterogeneus tumor class that includes cases that are phenotypically ADC despite the absence of obvious glands. AbstractThe major problem confronting the cytopathologist in interpretation of the pleomorphic aspiration biopsy sample (ABS) of nonepithelial lesions is to provide a reasonable differential diagnosis, since the presence of cellular pleomorphism alone does not always denote malignancy. One must be fully cognizant of a vast number of pathologic processes to make a correct interpretation. Some indication of the extent of the problem and the potential diagnostic pitfalls are outlined in selection of nonneoplastic processes and benign and malignant tumors of mesenchimal origin presenting as pleomorphic ABSs.

AbstractIn corneal scraping smears from 13 patients with clinically suspected herpes simplex keratitis (HSK), HSK is demonstrated by means of peroxidase‐antiperoxidase (PAP) technique with antisera to herpes simplex virus (HSV) in Papanicolaou‐destained cellular samples. The staining for HSV antigen was present in seven cases of corneal scraping smears with superficial keratitis (dendritic and geographic ulcers) while six cases of stromal keratitis (deep keratitis) failed to show HSV antigen except in one case. Specific antigen for HSV was predominantly present in the cytoplasm rather than in the nucleus. Immunoreactions were negative with HSV antisera in patients with other infections and in those in a normal control group. AbstractA case of recurrent esthesioneuroblastoma was diagnosed by fine‐needle aspiration of a midcervical mass. The cytologic features that characterize this uncommon neoplasm include uniform, small, round‐to‐oval cells with coarsely granular chromatin, multiple small nucleoli, prominent nuclear membranes, and scant cytoplasm. Morphologically and ultrastructurally, esthesioneuroblastoma cells resemble those of sympathetic neuroblastomas. Both tumors show rosette formation and minimal cellular pleomorphism, and, by electron microscopy, dense‐core cytoplasmic granules can be identified. The cytologic recognition of esthesioneuroblastoma is important since it metastasizes to cervical lymph nodes accessible to fine‐needle aspiration biopsy.
