AbstractFine‐needle aspiration cytology (FNAC) was performed on 195 cases of salivary gland lesions. The smears were technically adequate in 178 cases. Tissue examination was available for subsequent histocytologic correlation in 57 cases. The cytodiagnosis included inflammatory lesions (59) and benign (68) and malignant (51) tumors (total, 119). The accuracy of cytodiagnosis was 87.7% with a sensitivity of 80.9% and a specificity of 94.3%. Exact histologic typing was possible in 61.9% of the malignant tumors. AbstractCertain authors state that primary genital herpes infections mainly produce cells with ground‐glass nuclei, whereas secondary infections produce a higher percentage of multinucleated or single cells with prominent intranuclear inclusions. The cervicovaginal smears of 11 patients with primary genital herpes infection and 16 patients with secondary infection were studied to determine the validity of this differentiation. The percentage per patient of virally infected cells containing distinct inclusions ranged from 0% to 35% in the primary herpes group and 0% to 100% in the secondary herpes group. In eight additional patients, the initial herpes simplex virus (HSV)‐positive smear was compared with recurrent positive smears taken from 2 to 120 mo later. No statistical difference in the incidence of distinct inclusions in the infected cells was detected. Finally, primary rabbit kidney and the heteroploid A549 cell cultures were inoculated with herpes viruses and followed by fixation with several agents at various time intervals. AbstractThis study assessed the morphological criteria for the diagnosis of various types of lymphocytic thyroiditis in fine‐needle aspirates. Of 950 aspirates, 121 revealed lymphocytic thyroiditis, including Hashimoto's thyroiditis (partly confirmed by serological or histological examination) and focal thyroiditis adjacent to neoplasms. The diagnosis of Hashimoto's thyroiditis was easy when the aspirated material was adequate and contained oxyphilic cells: in the fibrous type, diagnosis was rather dificult. Focal thyroiditis may be confused with Hashimoto's thyroiditis, especially when adjacent to neoplasm. Surgical exploration should be performed in cases of severe lymphocytic thyroiditis revealed by fine‐needle aspiration with repeatedly negative antibody titers in order to exclude neopl

AbstractEndoscopy significantly improves the diagnosis of tumors of the stomach: however, its value may be limited in differentiating benign from malignant gastric ulcers (MGUs). The accuracy of endoscopy was compared in 20patients with MGU studied by endoscopy, biopsy, and cytology. Endoscopy correctly diagnosed 13 cases (65%), with endoscopic signs being benign or indeterminate in the remaining seven. However, all 20 MGUs were accurately diagnosed since biopsy and cytology increased the diagnostic accuracy of endoscopy in the remaining seven patients. The combination of biopsy and cytology yielded a positive diagnosis in 19 patients (95%); the remaining patient was operated on with only the endoscopic diagnosis of MGU. The complementarity of these three techniques was highlighted in five cases where only one of the procedures unequivocally diagnosed malignancy. It is concluded that biopsy and cytology are useful complementary techniques and should always be performed together in categorizing the nature of all gastric ulcers despite a benign macroscopic appearance at endoscopy.

AbstractAspiration specimens from 12 patients with histologically documented ductal carcinoma‐in‐situ (DCIS) of the breast (seven patients) or DCIS with minute foci of stromal invasion (five) were evaluated. Five patients presented with palpable masses, 1.5–4.0 cm, and four patients presented with localized thickening, associated with nipple erosion and discharge in two of them. One patient had nipple inversion, and one patient had bilateral nipple discharge. In one patient, no apparent abnormality of the breast was present. Mammography was either suspicious for or strongly suggestive of carcinoma in 10 patients and negative in two. Aspirates from all patients were composed of fragments of atypical ductal epithelium and numerous single epithelial cells. In nine cases, the smears were hypercellular and similar to aspirates of typical invasive ductal carcinoma. Calcifications were present in six cases. In four of these, associated tumor necrosis was evident. Cytologic features separating DCIS patients from those showing minimal stromal invasion or common types of invasive ductal carcinoma (IDC) were not identified. We conclude that fine‐needle aspiration cytology of DCIS is identical to that of IDC. AbstractDuring the last 4 yr. fine‐needle aspiration cytology (FNAC) has been employed in 1,474 patients in 0‐15‐yr age group at our institute. Of these, 245 patients were found to have malignant disease, including primitive neuroectodermal tumors, hepatoblastoma, nephroblastoma, sarcoma, and epithelial malignancies. AbstractAspirates obtained from bone marrow were prepared for routine Papanicolaou staining and screened by a cytologist. As a preliminary study, 100 consecutive bone marrow aspirations were examined. It was shown that metastatic carcinoma and primary bone marrow disorders (including leukemia, myelodysplastic syndromes, and multiple myeloma) can be recognized on Papanicolaou‐stained marrow aspirates using the same cytologic criteria employed in the evaluation of cells from any site. AbstractThe cytologic results of 44 consecutive fine‐needle aspiration (FNA) biopsies of the pancreas are reported. The series consisted of 27 women and 17 men with an age range of 31–89 yr (mean, 61.5), Excluding insufficient cases, the sensitivity of the procedure was 88%. specificity was 100%, positive predicative value (PV) was 100%, negative PV was 69%, and efficiency of the test was 90%, There were 29 true‐positive, four false‐negative, and nine true‐negative diagnoses. Two specimens were insuficient for diagnosis. Giant cells of varying types were seen in both the malignant and benign cases. Two of the benign cases demonstrated rare multinucleated foreign body‐type giant cells, most likely representing the changes seen in pancreatitis. In 13 malignant cases, multinucleated tumor cells were present, while six additional cases had multinucleated benign histioeytes reflecting the associated pancreatitis. Two malignant cases each had tumor giant cells and benign multinucleated histiocytes. Three of the malignant cases had numerous multinucleated tumor giant cells arranged in a dissociative fashion with evidence of cytophagocytosis consistent with a pleomorphic giant‐cell carcinoma of the pancreas. One additional case demonstrated numerous multinucleated osteoclastic‐like cells consistent with an osteoclastic tumor of the pancreas. This article documents the accuracy of FNA biopsy of the pancreas and notes that giant cells of varying types can be found in pancreatic FNA biopsies. AbstractFine‐needle aspiration cytology was performed in two cases of intra‐abdominal tumors, one from retroperitoneal lymph nodes and one from a cryptorchid testis. The cytological diagnosis was consistent with seminoma in both cases. Cytological features included uniform neoplastic malignant cells with round nuclei and nucleoli. AbstractMetaplastic carcinoma of the female breast is an uncommon lesion that may metastasize to body sites. To our knowledge, this is the first reported case of pulmonary metastasis diagnosed on bronchial brush cytology. The patient presented with pulmonary symptomatology 1 yr and 3 mo after a left modified radical mastectomy for breast carcinoma. Bronchoscopic examination revealed an endobronchial lesion partially obstructing the bronchus intermedius. Bronchial brush cytology showed the presence of nonkeratinizing squamous tumor cells admixed with a few scattered clusters of tumor cells with glandular features. Histologic review of the original breast lesion showed a metaplastic carcinoma of the breast with a predominant squamous‐cell component.
