AbstractThe relationship between plaque and inflammatory periodontal disease is complex, and requires extensive and often abstruse research. One unfortunate consequence is that much relevant information concerning this relationship is not readily available to those most concerned with the prevention and treatment of inflammatory periodontal disease. The present review is concerned with some relevant developments in recent plaque research.

AbstractAn apparatus for recording tooth mobility of maxillary anterior teeth using loads below 100 p in a bucco‐lingual direction is described. Strain gauges and a differential transformer are used as sensors of force and displacement. The displacement is recorded at the same location on the tooth surface and in the same direction as the loading force. Compressed air is used as the source of force. Documentation of signals is obtained by means of a two‐channel potentiometric recorder.Tooth mobility was studied in six subjects with healthy periodontal conditions and in six subjects with moderate periodontal disease. Repeated measurements under standardized conditions demonstrated a satisfactory reproducibility of the measuring method. Tooth mobility curves within the 20–80 p loading range were transformed to lines of regression. Analysis of such lines seems to provide further information of tooth mobility qualities as compared to previous methods of tooth mobility measure

AbstractA family study was carried out, based on the hypothesis that juvenile periodontitis is inherited in an autosomal recessive mode. Propositi were selected according to standardized criteria. Thirty‐one cases with a female/male ratio of 20 to 11 were obtained. Their first‐degree relatives were examined radiographically, mostly by orthopantomography. Sixty parents. 64 siblings (out of 66) and three children aged 13–16 were traced and examined. Neither parents nor the children showed findings suggestive of juvenile periodontitis, whereas 11 of the 64 siblings were affected.The genetic ratio was first calculated by using ana priorimethod assuming complete ascertainment. The result, 0.256 ± 0.061, is close to the theoretical assumption of 0.25. Since the calculation was based on the assumption that juvenile periodontitis is inherited as an autosomal recessive trait, the results were also exposed to ana posteriorianalysis assuming a truncate, complete ascertainment. The result. 0.268 ± 0.062, is again close to the expected ratio for autosomal recessive inheritance. A third method assuming a single, very incomplete ascertainment gave a much lower ratio. AbstractA study was undertaken on a group of patients who have had periodontal treatment in an attempt to test some of the predictions of the “Health Belief Model” (Rosen‐stock et al. 1966). This present study set out to test whether (1) persons who knew they had periodontal disease (hence considered themselves susceptible) (2) thought the condition serious and (3) had a knowledge of how to prevent the disease (4) were effectively complying with a previously prescribed oral hygiene regimen.The study was conducted at a school of dental hygiene, providing a source of patients attending specifically for periodontal care and where the format of hygiene instruction and treatment is largely standardized.A questionnaire was designed to assess (1) attitudes and beliefs (2) knowledge of the disease (3) reported behavior. Gingival health was measured using Gingival Index (Löe&Silness 1963) and patients actual behavior was measured using the Plaque Index (Silness&Löe 1964).The majority of the highly motivated patients in the study were concerned with their susceptibility to, the seriousness of, and the benefits of treatment for periodontal disease. They were stimulated to take action by a cue. bleeding gums. These findings confirm the predictions made in the Health Belief Model.However, none of the factors in the Health Belief Model were significantly associated with the gingival health status of the patients, indicating that other factors were important for patients to clean their teeth effectively. AbstractTeeth destined for extraction and exhibiting periodontal pockets had reference grooves cut on the proximal surfaces at the level of the gingival margin. A disclosing agent was then applied either as a mouthrinse or by direct irrigation at the entrance of the pocket. After extraction, the extent of apical penetration of the solution was measured from the reference point. Subsequently, the tooth was stained enabling similar measurements from the reference point to the level of the apical plaque border and the coronal level of the residual periodontal ligament.Analysis of these measurements indicated that neither mouthrinsing nor direct irrigation routinely achieved penetration of the solution to the apical plaque border. While mouthrinsing failed to achieve any significant penetration of pockets, the direct irrigation technique was partially effective.It was concluded that direct irrigation offers promise as a mode of application of chemical agents to subgingival plaque.

AbstractA double‐blind crossover trial was carried out to assess the effect of zinc citrate on plaque accumulationin vivoand to measure total salivary aerobes and anaerobes both during the trial and throughout the day following a single rinse. When compared to the placebo, zinc citrate significantly reduced the plaque accumulation in the subjects. However, the mean reduction was small and of qestionable clinical importance. A progressive increase in plaque was observed throughout both active and placebo 7–day rinsing periods. AbstractThis study was designed to determine the necessity for and effectiveness of the free autogenous gingival graft in maintaining the periodontal attachment to teeth. Ninety‐two patients with bilateral facial surfaces of inadequate keratinized gingiva had a free autogenous gingival graft placed on one of the facial surfaces (experimental side) while the contralateral side served as the control. Measurements were recorded from the CEJ to the 1) free gingival margin, 2) base of the gingival sulcus, and 3) mucogingival junction on the mid‐facial of selected teeth. The Gingival Index and Plaque Index (Löe 1967) were also evaluated. A total of 107 grafts were performed and followed from 6 months to 2 years post grafting. Patients were recalled (scaling, root planing, plaque control instruction) and clinical parameters such as pocket depth, recession and attachment level were measured at 3‐ and 6‐month intervals. Both experimental (grafted) and control (unoperated) sides showed marked reduction in the Gingival Index and Plaque Index. Neither side demonstrated further attachment loss. It would appear that minimizing inflammation is sufficient to maintain attachment levels despite the width of keratinized attached

AbstractIn four beagle dogs, periodontal tissue breakdown was experimentally induced around the upper left premolars (P1, P2and P3). Wooden probes were inserted mesially and distally to each premolar. Six different probing forces were used in each dog. Microscopic examination showed:1) the presence of an intact layer of epithelium between the probe tip and the connective tissue in 21 of 23 specimens.2) an increasing compression of the connective tissue with increasing probing force, without penetration of the probe tip into the connective tissue.
