AbstractThe present study compared surgical therapy to root planing atone in the treatment of periodontal intraosscous defects. 25 defects in 14 patients were subjected to root planing only and another 25 defects in the same patients were surgically exposed and citric acid treated. The healing response was evaluated 6 months after treatment The mean gain of probing attachment level was 0.8 mm in the root‐planed defects as compared to 1.3 mm for the surgically exposed and acid‐treated defects. The probing bone level improved an average of 0.2 mm for the root‐planed areas as compared to 0.6 mm for the acid‐treated defects. The mean preoperative probing pocket depths of 6.7 mm and 6.7 mm for the 2 groups were reduced to 5.2 mm and 4.1 mm, respectively. Abstract106 sites with probing pocket depths 7 mm or greater from 14 patients were treated with plaque control instruction and I episode of root planing. Sites in each patient were either irrigated with 2% chlorhexidine or left as non‐irrigated controls. Irrigation immediately followed root planing and was repeated daily, by the patient, for 24 weeks. Clinical measurements were made at 12 and 24 weeks, as were gingrval washings for determining the number and % of spirochetes. Results at 24 weeks demonstrated that bleeding scores decreased from 91% to 9%; the % of spirochetes dropped from approximately 9% to less than 1 %; probing pocket depths decreased from 7.5 to 4.5 ram, and probing attachment levels gained 1.1 to 1.4 mm. The chlorhexidine irrigated experimental group and the non‐irrigated control group did not differ significantly in any of the studied parameters. AbstractThe aim of this study was to determine whether changes in probing attachment levels are related to subgingival spirochete or leukocyte counts in periodontal pockets. Following initial clinical recordings and therapy consisting of oral hygiene instruction and root debridement, the probing attachment levels of proximal sites of 120 single‐rooted teeth in 7 patients were measured every 3 months for 12 months. The measurements of each site were subjected to regression analysis, which determined whether the site was deteriorating, improving, or non‐changing. Subgingival washings were taken of 19 deteriorating, 22 improving, and 127 non‐changing sites to determine the number and % of spirochetes and the number of leukocytes at each site. Improved probing attachment levels were associated with reduced numbers of spirochetes and leukocytes. However, the ranges of individual measurements of subgingival washing variables overlapped considerably between groups. Spirochete and leukocyte counts related better to the 12‐month probing depths than to changes in probing attachment levels during the preceding 12 months. AbstractApical subgingival plaque samples were taken from 19 subjects exhibiting active destructive periodontal disease. The predominant cultivable Gram negative species from 50 active sites were compared to 69 inactive sites of comparable pocket depth and attachment level loss. Active disease sites were chosen which showed a significant loss of attachment within a two‐month interval.Proportions of Gram negative rods were higher in active periodontal disease sites than in inactive sites. Species which were found to be significantly elevated only in active sites wereBacteroides intermedius, “fusiform”Bacteroides, Actinobacillus actinomycetemcomitans, andWolinella recta. AbstractPlaque was collected from a group of volunteers who used either Listerine antiseptic (LA), its vehicle control (V), or a water control (C) twice daily in addition to their normal tooth brushing in a double‐blind controlled clinical experiment. Following the 9‐monlh clinical study, plaque collected from the supragingival surfaces of 20 teeth from each of 78 subjects was weighed wet, freeze dried. reweighed, resuspended, sonicated and estimated for protein. In addition, endotoxin activity was evaluated by means of the limulus lysate assay. A 52.6% reduction in wet weight was found LA versus C (p= 0.04); LA versus V showed a 55.1% reduction (p= 0.03). A 59.0% reduction in dry weight was found LA versus C (p= 0.01); LA versus V showed a 59.6% reduction (p= 0.01). A 59.7% reduction in plaque protein was seen LA versus C (p= 0.01); LA versus V showed a 59.2% decrease (p= 0.02). A 75.8% reduction in limulus lysate activity was found LA versus C (p= 0.01); LA versus V showed u decrease of 77.9% (p= 0.01). Abstract32 patients with bilateral areas of inadequate attached gingiva on the facial surface of homologous contralateral teeth have been followed for 6 years. Treatment consisted of scaling, root planing, oral hygiene and maintenance at 3‐ to 6‐month intervals or as needed to control inflammation. A free gingival graft was placed on one side (experimental), while the other side served as the unoperated control.Areas of inadequate attached gingiva on control sides, to include those with recession and no attached gingiva, didnotdemonstrate additional recession or further loss of attachment. Gingival inflammation and plaque were significantly reduced. On experimental sides, the dimension of keratinized and attached gingiva increased and was stable over 6 years. Areas which began with recession and no attached gingiva exhibited a reduction in recession and gain in clinical attachment following the placement of a gingival graft.Examination of patients who had discontinued participation in the study for a period of 5 years revealed a reestablishment of gingival inflammation on the control sides associated with additional recession. Similar changes were not observed in areas treated by a free graft.The findings demonstrate that it is possible lo maintain periodontal health and attachment through control of gingival inflammation despite the absence of attached ging

AbstractThe effect of plaque control on the apical microflora of deep periodontal pockets was studied. 8 subjects exhibiting signs of chronic periodontitis were chosen for the study, each subject having at least one pocket 6 mm. These subjects were placed on a plaque control programme consisting of 3 visits, during which oral hygiene instructions were given. On two visits, the teeth of these subjects were scaled and polished. Bacteriological samples from the apex of a deep pockets from each subject were collected before the commencement of the plaque control programme and again at 8 and 16 weeks after the last scale and polish. No significant difference in the microbial flora was observed before and after plaque control, but marked fluctuation in bacterial composition was noted at the 3 samplings. It was concluded that supragingival plaque reduction was not sufficient to produce significant changes in the subgingival plaque composition of deep periodontal pockets.

AbstractThe purpose of this investigation was to examine the regeneration of alveolar bone following surgical and non‐surgical periodontal treatment. A total of 16 patients who had advanced periodontitis and demonstrated angular bony defects on radiographs participated in the study. After the initial examination, they received instruction in oral hygiene and had their teeth thoroughly scaled. When the individual patient at 2 succeeding appointments had plaque on less than 20% of the tooth surfaces, one maxillary and one mandibular quadrant was treated with the modified Widman flap procedure while one of the remaining quadrants was treated with the reverse bevel flap procedure. The last quadrant was treated with root planing under local anesthesia. None of these procedures included bone contouring. Following treatment, the patients were recalled every 2 weeks for professional tooth cleaning. Radiographs taken 12 months following treatment revealed that only minor changes in the hone level had occurred in areas with horizontal bone loss following the various treatment modalities. Following the modified Widman flap procedure, however, a statistically significant coronal regrowth of bone (0.5 mm) had occurred in angular bony defects. AbstractA 9‐month double‐blind controlled clinical study was conducted on adult subjects using either Listerine antiseptic, its vehicle control, or a water control in order to determine the efficacy of the antiseptic mouth rinse in inhibiting the development of plaque and gingivitis. Following screening examinations for minimal entry levels of plaque and gingivitis, all subjects received a complete prophylaxis. Subjects then continued their usual oral hygiene habits for a 3‐week normalization period and were examined for soft tissue abnormalities and baseline measurements of plaque, gingivitis, and tooth stain. 2 additional prophylaxes were then performed, followed by a second baseline gingival examination. Zero plaque was re‐established by rubber cup polishing and twice daily rinsing was begun. Soft tissue, plaque, gingivitis, and extrinsic tooth stain were evaluated after 1, 3. 6 and 9 months of rinsing with the randomly assigned mouthrinses.Results demonstrated that Listerine antiseptic significantly reduced the development of plaque at 1,3.6 and 9 months and the development of gingivitis at 9 months, as compared to its vehicle control or water
