AbstractEarlier studies have demonstrated that, with similar levels of plaque accumulation, the tendency to develop gingivitis is lower in pre‐school children than in young adults. No studies are available in which the gingival reaction in older children and adults to a given amount of plaque has been compared. The aim of the present study was to compare the gingival reaction to dental plaque in children of different ages and in adults.3 groups of children (4–6. 7–9 and 14–16 years of age) and 1 group of adults (20–22 years of age) were studied. The degree of gingivitis was determined using the gingival index and the amount of dental plaque was assessed in terms of the plaque index.The results showed that with a given plaque score, the % of high gingivitis scores was less in the 4–6‐year‐old children than in the older children and adults. At all levels of plaque accumulation, the highest degree of gingival inflammation was noted in the 14–16‐year‐old children and in the adults. AbstractThe problems of periodontal and prosthetic treatment are demonstrated with a typical case of oral erosive lichen planus with gingival involvement. The results attained suggest that oral lichen can be improved by optimum plaque control and periodontal treatment. Prosthetic treatment should be based on bridgework and not on removable partial dentures owing to the isomoarphous irritant effect that characterizes lichen planus. Once the plaque situation is firmly under control, even teeth with advanced destruction of the supporting tissues may be used as abutments. In the event of all molars being absent, cantilever bridges should be fitted. In this case it is not essential to replace all the molars provided that the masticatory function is subjectively adequate.

AbstractThe composition of subgingival plaque, from healthy and periodontally diseased regions, as well as the clinical periodontal condition of 6 patients, 17 to 24‐years‐old suffering from juvenile periodontitis were examined. 6 older patients with rapidly progressive periodontitis served as a control. Samples of subgingival plaque were taken from first molars and central incisors and were analysed morphologically by dark‐field microscopy. In the control group in healthy regions the ratio between non‐motile and motile bacteria was 27‐1 and about 1:1 in deep pockets. In the juvenile periodontitis group in diseased regions, motile bacteria made up only 1/1 of the bacteria present. The results clearly show that our relatively old patients with juvenile periodontitis had a completely different microflora in their periodontally diseased regions than patients with common periodontitis. As regards our observations by dark‐field microscopy, one can assume, however, that with increasing age there might be a shift of the flora associated with juvenile periodontitis lesions from a rather simple composition to a more complex composition very similar to the flora seen in rapidly progressive adult periodonti

AbstractThe etiology and pathogenesis of juvenile periodontitis may involve dysfunctions of the host response. In particular, the neutrophil and the lymphocyte have been implicated in the disease. The purpose of the present study was to examine the in vitro spontaneous lymphocyte response and neutrophil chemotaxis in populations of localized juvenile periodontitis (LJP) and generalized juvenile periodontitis (GJP) patients and age‐ and sex‐matched healthy subjects (HS). These laboratory values were also evaluated immediately following and 1 year after periodontal therapy. The results show that spontaneous lymphocyte responses reflecting the autologous mixed lymphocyte reaction (AMLR) are depressed for GJP patients. The decreased AMLR in the GJP group appears to represent an abnormal T‐cell function which may reflect activity of the periodontal lesion. LJP patients have an increased AMLR response, although h was not statistically significant. I year following active periodontal therapy, spontaneous lymphocyte responsiveness returned to normal in most GJP patients. The increased spontaneous lymphocyte responsiveness of LJP patients was not changed either immediately following active periodontal therapy or 1 year later, LJP and GJP patients exhibited a neutrophil chemotaxis defect when compared lo cells from HS. AbstractThe uneven distribution and pattern of staining, revealed on root‐planed, periodontally‐involved root surfaces by the application of a plaque‐disclosing solution, precludes the use of conventional plaque scoring indices or other methods of assessment based on planimetric and gravimetric techniques. A method has been developed and evaluated to quantify stained areas using image analysis. The method is reproducible, with an error factor of 1.32%, and represents a comparatively simple yet effective technique for quantifying plaque on irregular root surface

AbstractThe importance of adequate root surface instrumentation has received increasing emphasis. The purpose of this study was to determine the extent to which root planning could produce surfaces free of stainable deposits. Initial laboratory investigations on extracted, periodontally involved roots demonstrated that after meticulous root preparation, totally non‐stainable surfaces could be obtained. These surfaces were shown to consist of either thin cementum or dentine.The efficacy of instrumenting periodontally involved buccal root surfaces on the anterior teeth of 33 instrumented either before or after the reflection of surgical flaps. Remaining bacterial deposits were disclosed with a gentian violet solution and their root surfaces then photographed. Further root planning, disclosure and photography were then carried out. These photographic slides were analysed for stainable deposits on the root surfaces using an image analysis system, based on densitometric principles, to measure the areas of stainable root surface deposits. The findings revealed that root planning under direct vision at the time of surgery was more effective than blind instrumentation. AbstractA previous study of lactate dehydrogenase (LDH Jin gingival crevicular fluid (GCF) suggested that the concentration is 10 to 25 times that of serum (X=2300 international units/I versus 100 IU/1 for serum). That study used capillary tubes to collect microliter amounts of GCF. Since invasive collection techniques can influence GCF flow, we evaluated LDH activity in GCF collected by filter strips. GCF was collected in a standardized fashion from 10 subjects with mild inflammation (GI=0.5–1.0) and 10 subjects without evidence of gingival inflammation (GI=0). Our results indicate that LDH volume activity was greater for subjects with GI=0 x= 105.529 IV/l) than for subjects with GI=0.5–1.0 (X;=77,66 1 IU/1). but the difference was not significant. LDH total unit activity was significantly greater in subjects with GI=0.5–1.0 versus GI=0 (X=0.048 IU versus X=0.0242 IU.P<0.0001). AbstractThis report details some preliminary observations regarding cell morphology and behavior during early phases of invivoattachment to demineralized surfaces. Rectangular dentin specimens prepared from beneath calculus‐covered areas of root surfaces were treated with citric acid and implanted vertically into the skin of rats such that one end of the implant protruded above the skin. The implants were removed after 1 day and the dentin surface—connective tissue interface examined using scanning electron microscopy. A dynamic series of biological events seemed to be in progress.
