Contraction of fibrous capsules is the most common complication of augmentation mammaplasty using silicone implants. This contraction occurs unpredictably with all kinds of implants, and despite various surgical maneuvers to prevent it. Current clinical practices are reviewed.Wound healing around silicone breast implants is similar to that elsewhere in the body. Acute inflammation is followed by collagen synthesis. Breast capsules contain contractile fibroblasts like those found in other contracting scarsSilicones implanted into tissue undergo some detectable changes that at present cannot be related to capsular contraction. The possible roles of physical and chemical characteristics of the silicone implants are discussedSteroids are widely used to prevent contraction, but much debate exists over whether or not they actually work. Other possible drug treatments are discussed, based on current research.

The usual cross-leg flap procedures to cover composite wounds involving exposed ankle joint or bone require awkward positioning of the extremities with resultant complications. This is especially true in young, muscular patients with thick legsA dorsalis pedis flap from the uninjured foot used as a cross-foot flap has been reliable and successful in our experience. A representative case history and the multiple advantages of this technique are described. We believe that this flap is an excellent alternative to the free-flap technique for coverage around the ankle area, especially in hospitals in which facilities for free-flap technique are not available

This lesion can be encountered frequently by the plastic surgeon; its histology, incidence, and treatment are reviewed here. A recently introduced polymer preparation (Iodoplex) is described. It is based on a new type of hydrophilic polymeric carrier into which iodine can be incorporated to the extent of 50% or more. Its main feature is that the polymer and the petroleum jelly base are water-insoluble. The iodine is slowly released from the polymer by body fluids, which are rich in protein and dead cells. Iodine has been found to be useful in the treatment of donor sites; no pain or discomfort attends its use, and the dressing peels off easily from the healed epithelium. A controlled study comparing Iodoplex-impregnated and scarlet red—impregnated gauze on donor sites was carried out, and the use of Iodoplex was found to have some real advantages over scarlet red.
