A case of coccidioidal meningitis following an open-head injury is presented. A 6-year-old boy was ejected from a motor vehicle as it was driven over a cliff, resulting in a severe open-skull fracture with grossly contaminated wounds. The accident occurred in an area in which coccidioidomycosis is endemic, and the causative agent, Coccidioides immitis, is found in high concentration in the soil. In addition to fracture reduction, the child received a course of intrathecal and intravenous amphotericin and achieved a satisfactory clinical response.

Crescentic subcutaneous pedicle flaps prepared on the area lateral to the alar base were designed to cover the skin defects in and around the philtrum.In 1 patient a combination of the lateral lip advancement flap and the crescentic subcutaneous pedicle flap was applied to the skin defect on the lateral lip and philtrum. In another patient, a combination of two crescentic subcutaneous pedicle flaps was applied to the skin defect within the philtrum. Both methods have been successful.

An inferiorly based rectus abdominis musculocutaneous flap was used to close a large ischiosacral pressure sore refractory to the usual methods of closure in a paraplegic patient. The functions of the rectus abdominis muscle to initiate vertebral flexion from a supine position and aid in respiration, urination, defecation, and vomiting may play a proportionately greater role in the paraplegic patient. Although our patient did not experience any noticeable clinical deficits, this donor site is theoretically less functionally expendable in the paraplegic patient.

Multiple hereditary lipomatosis has been reported very rarely in the literature. The largest number of lipomas previously reported in a single patient was 160. We report on a 45-year-old man who began to experience multiple lipomas at about age 26. The lipomas multiplied until they severely disfigured his entire torso, arms, and upper legs. Both sides of his body were affected to approximately equal degrees. A family history revealed that the patient's father, two brothers, and two nephews have all experienced multiple subcutaneous lipomas, although to a lesser degree. In treating the patient, what appeared to be very large lipomas were in fact found to be subcutaneous clusters of hundreds of small encapsulated lipomas. Lipomas were excised using suction lipectomy to treat different involved areas on eleven separate occasions. A total of 8,475 cc was removed over a two-year period. A modified, slightly less blunt cannula was developed to aspirate the clustered lipomas individually.

A case of multiple symmetric lipomatosis in a 61-year-old man is described. The patient had the striking appearance that characterizes multiple symmetric lipomatosis. He had a history of alcohol abuse. Before admission to our institution, he underwent surgical treatment three times at other hospitals. We describe his clinical course and review the literature.

Effective stabilization of the endotracheal tube is required for safety when performing surgery in the craniomaxillofacial region. We describe a simple technique for rigid stabilization of the airway, which has been used in over 150 patients without complication.

The critical need to maintain closed-circuit airways during maxillofacial surgery has led to a number of innovations in anesthetic tube placement and stabilization. Several redesigns and alterations of endotracheal tubes have been described and are currently in use. Also several methods of securing endotracheal tubes have been described. These range from suturing the tubing to the patient's scalp to fabricating maxillofacial prostheses. In this report, a readily available and easy method of securing anesthetic tubes is described. This technique has been used successfully in hundreds of patients at our institutions.
