Fatty tumors of the peripheral nerves, although rare, occur with sufficient frequency to pose problems in diagnosis and treatment. Moreover, the nomenclature of these tumors has suffered from controversy over their morphological description. These factors have contributed to a misunderstanding about the treatment of these localized nerve abnormalities leading, in the past, to unnecessary radical nerve excisions with resultant total motor and sensory deficit. This paper reviews the various types of fatty tumors, describes the morphology and pathophysiology of one such lesion, and provides a basis for a rational surgical approach.

Plastic surgical operations are among the oldest and have been developed increasingly from ancient times down to the present. However, the age of medical specialization did not begin until the latter part of the nineteenth century, and the specialty of plastic surgery is largely a child of the twentieth century. Most of the operations we do today have been created by plastic surgeons with the past 50 years, but were made possible only by the rapid developments in other surgical specialties and in all of medicine and science.The relationship with other specialties has been an interdependent one, and this is likely to be the pattern of the future. Plastic surgery, bridging the anatomical specialties as it does, must continue to draw bits of information here and there from them and to synthesize these into major contributions of great benefit to all. The specialty will grow in direct proportion to the success of the innovators within it in fulfilling their crucial role.

The staged management of an 8-month-old boy with congenital tubular nostril, or proboscis lateralis, is described. Absent were the right frontal and maxillary sinuses, right nasolacrimal apparatus, vomer, and right lateral nasal wall. Present was a “baby elephant trunk” attached to the right medial canthal region.Attachment of the tubular nostril was carried out in two stages. A bipedicle with serrated edges was created from the soft tissues of the right medial cheek. Under this was buried the lower two-thirds of the tubular nostril previously deepithelialized circumferentially with a similar serrated outline. Five months later, the remaining upper third of the tubular nostril was deepithelialized, detached from its origin in the medial canthal region, and similarly buried under a bipedicle tunnel. Adjustments of the alar rim, alar base, and nasal tip were done by excisions and Z-plasty. Exostosectomy at the right medial canthal region provided further refinement.No retardation of growth was evident after 4 years.

One of the most difficult areas on the body to skin graft successfully is the interscapular and upper back region. We have used a wire “basket” to immobilize, the interscapular region in 8 patients following wide excision for melanoma. “Takes” amounting to more than 90% in 7 patients and 80% in 1 patient have been obtained. The technique is simple and inexpensive.

摘要:

The most recent methods of distinguishing between organic and psychological impotence are reviewed. The old theory that 90% of impotence is psychological is disclaimed and the various nonpsychological causes are pointed out. The majority of impotent patients can be treated in a nonsurgical manner.
