A 31-year-old female patient with both acquired immunodeficiency syndrome and a solid silicone buttock implant presented with fever. Using ultrasound-guided aspiration of the periprosthetic space, a diagnosis ofMycobacterium aviumcomplex infection involving the implant was made. The implant was surgically removed and the patient had a resolution of her fevers while treated with ethambutol, rifabutin, ciprofloxacin, and clarithromycin.

Lipohypertrophy is one of the local side effects of repeated subcutaneous Insulin injections. This case report demonstrates the successful cosmetic and therapeutic treatment of two lipohypertrophic masses in the lower abdomen of a young diabetic patient by suction-assisted lipectomy.

The 3M microvascular anastomotic coupling device has gained popularity because of its obvious advantages of speed and simplicity. Many reports attest to its safety and efficacy in microsurgery, including two reports on its use in the hand. We present 2 patients in whom the use of the 3M coupling device in the hand resulted in a palpable foreign-body sensation that was unpleasant to the patient and ultimately had to be removed.

Advanced knowledge about cutaneous vascularity has enabled the development of axial flaps such as fasciocutaneous, septocutaneous, and musculocutaneous flaps. It has also laid the foundation for microsurgical free flaps. The newly emerging concept of angiosomes sheds yet more light on the complexity and multiplicity of peripheral blood supply. We describe the use of an extended vertical trapezius flap carrying a segment of latissimus dorsi muscle with the underlying paraspinal fascia to close an infected and irradiated posterior fossa craniectomy and dural defect. This flap illustrates the elevation of an angiosomal block of tissues as an axial flap vascularized by the alternative major blood supply of one of its constituents. The potentials of this “angiosomal flap” are explored. A brief review of dural defect repair is also presented.

A case of coup de sabre, a linear form of scleroderma, is presented. Treatment consisted of soft-tissue expansion and autologous bone grafting to the forehead, a composite graft for alar reconstruction, and a scalp graft for eyebrow reconstruction. None of the linear scleroderma cases reported in the literature consisted of bony reconstruction.

In the setting of traumatic injury, obtaining suitable coverage to salvage a below-the-knee amputation has been performed by using a variety of tissue flaps, including the fillet of foot free flap. This flap is a free tissue transfer that utilizes the soft tissue of the foot for stump coverage. In this report, we present a 15-year follow-up of a patient with a fillet of foot free flap to demonstrate its applicability and long-term efficacy. We also discuss technical aspects for performing this procedure. In this follow-up, we have found the flap to be extremely durable, even when placed in the weight-bearing position of the stump, and also lacking many of the usual complications associated with the weight-bearing portion (e.g., blisters, ulcerations). We conclude that the fillet of foot free flap is an excellent salvage flap for providing long-term length, coverage, and durability in an amputated, weight-bearing extremity.

Although liposarcoma is one of the most common soft-tissue sarcomas, facial localization is extremely rare. The buccal fat pad is an important anatomic structure located in the face that recently gained interest as a result of increasing research on facial anatomy. In this paper, we report a case of giant liposarcoma originating from the buccal fat pad. The precise localization of the tumor was determined preoperatively with computed tomography examination. The liposarcoma that invaded the body and the extensions of the buccal fat pad was resected completely. The pathological examination revealed a sclerosing, well-differentiated liposarcoma, which is known to be very rare in the head and neck region. Chemotherapy and radiotherapy were not necessary because of the favorable histological type of the tumor and the advanced age and poor general condition of the patient. Local recurrence and distant metastasis were not observed during the 1-year follow-up.

Apert's disease is distinguished from other acrocephalosyndactylys by bilateral and symmetrical deformities, including symphalangism of the hands, which is commonly present in the index, long, and ring fingers. In the correction of the Apert hand, simple separation and skin grafting may be used to create a five-digit hand. However, this technique may result In narrow, atrophic fingertips, and pulp pads. In this paper, a simple technique to create nail folds and pulps for each finger with four transposition flaps from the common pulp is described. During the past 3 years, four Apert hand deformities in 2 patients were treated with this procedure. At a mean follow-up of 32 months, good results were obtained.

Wearing eyeglasses following nose surgery is a problem faced by many patients who require glasses for clear vision. The solutions offered for this seemingly minor problem are unsatisfactory and usually mandate special spectacle frames. We developed a simple apparatus, using a baseball cap and a tie clip, that enables suspension of eyeglasses with no pressure on the nasal bridge. This solution utilizes readily available components and maintains clear vision with possible frequent readjustment of the glasses. It also avoids contact of the glasses with any part of the face, namely the operative site, dressing, and wound discharge. We find this technique a simple and useful solution for a disturbing problem in plastic surgery.
