Regional node dissection is practiced as a measure of prophylaxis in patients with stage I and II malignant melanoma. Although the drainage pattern of the extremities is obvious, in the head and neck and trunk it may be ambiguous.We have used lymphoscintigraphy to assist in delineating the lymphatic drainage in 22 patients with primary malignant melanoma. Fourteen patients had melanoma in the head and neck region, and eight had melanoma in the trunk region. Based on Clark's classification there were ten level III melanomas, eight level IV melanomas, and two level V melanomas; the levels of the remaining two melanomas were unspecified. Seven melanomas were between 0.76 and 1.5-mm thick, eleven were between 1.51 and 4.0-mm thick, and two were over 4.0-mm thick (the remaining two were unspecified). Regional nodes were clinically negative in 18 patients. The scan distribution was unexpected in 13 patients (59%), and it influenced the surgical procedure in 11 patients (50%).No patient incurred an adverse effect from the scan. We conclude that lymphoscintigraphy may be of value in guiding prophylactic lymph node dissection in melanoma patients.

This is a report of a failure of tissue expansion in a 43-year-old patient suffering from a mild form of osteogenesis imperfecta tarda. Filling of the expander was not possible. Skin histology showed thrombosis of the blood vessels and vessel wall necrosis. This finding supports the theory that tissue expansion does not depend as much on mechanical skin laxity as on the proliferative potential of the skin.

Ameloblastoma of the jaws is most often considered by clinicians to be a benign tumor with a distressing tendency for local recurrence. However, as the case presented here demonstrates, even in a patient without the characteristic history of multiple recurrences and operations, ameloblastoma may appear as a metastatic lesion, usually to the lung, many years after treatment of the primary tumor. This case is discussed, and the relevant hterature concerning metastatic ameloblastoma is reviewed.

Posttraumatic condylar hyperplasia can result in complex facial asymmetry composed of degrees of condylar and ramus overgrowth, malocclusion, and complementary maxillary deformity. Three patients with unilateral condylar hyperplasia are described to illustrate the scope of surgical and orthodontic treatment required to restore facial symmetry. Condylar hyperplasia with facial asymmetry should be recognized as a possible consequence of condylar injury, and further delineation of the onset and natural history of this deformity could lead to investigations or intervention before the establishment of all the components of the deformity.
