Symptoms of breast hypertrophy are discussed. Both physical and psychological indications for corrective procedures are related to patients' expectations. Shape is discussed in terms of the patient's preference, her activities, and her geographic and historic background. Two main techniques are described—the Arié-Pitanguy-Ribeiro technique with a dermis-fat flap, which is recommended for extremely ptotic breasts, and the oblique technique, which is preferred for rounder hypertrophic breasts. In small ptoses the original Arié vertical wedge excision technique is still indicated. All patients are treated by the Schwarzmann maneuver with preservation of the periareolar nerves and vessel network, and areola transposition at the end of the operation. No drains are used.

The author presents a simple, safe technique for reduction mammaplasty, with two variations, based on breast shape, hardness of tissue, and volume. It does not interfere with the physiology of the breast and can be performed easily by any surgeon experienced with reduction mammaplasty.

Three surgical techniques were employed in patients with craniofacial dysostosis: single frontoorbital advancement and inversion of the frontal bone (craniostenosis of the coronal suture); total frontoorbital advancement and inversion of the frontal bone (Crouzon syndrome); and total frontoorbital advancement, reduction of the interorbital distance, and inversion of the frontal bone (Apert syndrome). We were able to improve exorbitism and remodeling of the frontal area. Very little bone grafting is done in these techniques.

In patients with moderate laxity of the facial skin but with many fine wrinkles, only a facelift or only a chemical peel gives poor results. Some patients don't like the delay necessary when the two procedures are done separately. Yet to do them together is often dangerous for the skin. I propose a different tactic, in which the two procedures are done simultaneously and safely.

Experience with the use of free flaps for lower extremity reconstruction during the period 1973 to 1980 is reviewed. From 1973 to 1977, 12 groin and 2 deltopectoral flaps were used, with 8 successes and 6 failures. From 1978 to 1980, 19 myocutaneous flaps (15 latissimus dorsi, 3 tensor fascia lata, and 1 osteocutaneous groin flap based on the deep circumflex vessels) were performed, 17 successfully and 2 unsuccessfully.The reasons for the improved results seem to be the longer and bigger vascular pedicle, the use of more end-to-side arterial anastomoses, and the choice of arterial trunks that are more distal and easier to work with. For these reasons, myocutaneous free flaps are the flap of choice for microsurgical soft tissue reconstruction of the lower limb.
