The functional or aesthetic burn deformity invariably is the consequence of a local skin shortage. The ideal solution would be replacement with new normal tissues, and these can now be created via an adaptation of the natural phenomenon of tissue expansion. Representative examples of hypertrophic scars, skin grafts, and in particular burn alopecia are presented to demonstrate the superior improvements previously not possible with conventional techniques. The concept of tissue expansion deserves an important place in secondary burn reconstruction.

Skin deepithelialization is an integral part of many reconstructive procedures, but it can be a tedious and timeconsuming ordeal when using conventional techniques. The carbon dioxide laser has been used in rats to facilitate skin deepithelialization with no adverse effect on the expected wound healing. These findings have been extrapolated for clinical applications in allowing rapid creation of the dermal pedicle required for mammaplasty.

In an experimental study in rats, a piece of dorsal skin was removed, molded, and tanned in glutaraldehyde. After rinsing in saline, the piece of skin was tested for extensibility and implanted under the dorsal skin of the same rat. It was removed after 1 to 12 weeks and its extensibility was again measured. Specimens were examined under the light and electron microscopes. None of the 63 implants extruded and at 12 weeks a thin, densely adherent fibrous capsule had formed. Light and electron microscopy revealed early inflammatory changes that later subsided. The implant maintained or increased its stiffness. The weight of the implant remained constant.

Large trochanteric pressure sores can be reconstructed with the superolateral advancement of the distal gluteus maximus-posterior thigh myofascial cutaneous flap. The flap has a dual blood supply derived from the gluteal system and the deep femoral artery. This makes the distal gluteus maximus advancement flap very reliable and versatile. It can be designed on the musculocutaneous perforators of the gluteal system alone or on its dual circulation. It has several advantages over previously described flaps for trochanteric pressure sores.

A primary Orticochea pharyngoplasty with a primary palatoplasty was studied in 15 children with cleft palates. Although all children demonstrated velopharyngeal competence, radiographic, endoscopic, and oral examinations suggested that 54% of the children studied would not have needed a pharyngoplasty. In addition, 27% of the children demonstrated aberrant speech patterns associated with velopharyngeal incompetence. Emphasis is placed on the difficulty in identifying which cleft palate patients will have residual velopharyngeal incompetence and the need for continued study and development of techniques for more accurate prediction of which patients would benefit from a primary pharyngoplasty.

The purpose of this study was to investigate nose maturation by cephalometric sagittal surface area methodology longitudinally in males and females. Both males and females exhibited a statistically significant continuum of nose growth from ages 6 to 18. Growth ceased in both sexes at age 18. Correlation coefficients indicated that males' noses tended to be either large or small in all dimensions from ages 6 to 18. The same was not true for females. A major finding of possible clinical importance was that maturity is reached at age 12 in girls with 84% of adult area, and in boys at age 15 with 87% of adult size.

Recurrent subareolar abscess is an infrequent but troublesome disease of the breast among young women. Recent experience with 5 patients indicated that successful treatment of recurrent subareolar abscess requires excision of the diseased ducts and necrotic cavity, including the inverted section of the nipple. The operative procedure described herein allows us to radically manage recurrent subareolar abscess and at the same time leave a nipple with a good cosmetic appearance.

Two patients with leiomyosarcomas are described, both men with lesions located in the upper trunk. Both lesions were present for more than 10 years. The diagnosis was established after excisional biopsy. The diagnostic criteria are a tumor size greater than 2.5 cm in diameter; a mitotic rate of 1 or more per 5 high-power fields in cellular areas; and electron microscopic verification of smooth muscle cells. The literature is reviewed and the clinical presentation of the 2 patients is discussed. Wide excision is the treatment of choice after histological diagnosis.

A case report of a cystic preauricular mass that changed in size and position with jaw movement is presented. This tumor proved to be a ganglion of the temporomandibular joint (TMJ) cyst, which is relatively rare. When a preauricular mass retrudes into the masseter muscle in jaw opening, pathological conditions associated with the TMJ should be considered. Ganglion cysts of the TMJ are a benign pathological entity which can mimic parotid tumors. Temporomandibular joint radiography and perhaps ultrasonography of the region are useful in the preoperative evaluation of preauricular masses. The TMJ ganglion cyst can be successfully treated by direct excision and repair of the joint capsule. This approach avoids the potential morbidity of a superficial parotidectomy.
