摘要:
The modified Abbe flap procedure is indicated in secondary unilateral cleft lip in which the Cupid's bow and philtral complex of the cleft side have been destroyed as the result of previous faulty surgery. The Abbe flap is inserted in the center of the tight upper lip with or without prior scar revision, which may be done several months beforehand. The recipient pocket for the flap is made by dividing the upper lip into two parts at the line of the philtral ridge in the noncleft side and bringing the whole prolabial tissue with the residual lateral lip to the cleft side of the upper lip. The natural midline groove included in the lower lip flap is enhanced by a traction suture and simulates a philtral dimple with the split philtral ridges on both sides. Various other techniques are used to improve the final result.

摘要:
This paper deals with the unique aspects and surgical indications of “dish face” deformity, including “flat face” deformity, due to cleft lip and palate. The importance of deformities of the nose and lip caused by cleft lip and palate and the role of their reconstruction in the treatment of “dish face” deformity are described in detail. For the protruding chin and pseudoprognathism, vertical osteotomy of the ramus of the mandible is recommended over advancement osteotomy of the hypoplastic maxilla.

摘要:
This paper describes a new method of creating a natural anterior hairline using microsurgical free scalp flaps. This operative method is a one-stage procedure and the new hairline has normal hair density and natural hair direction. The donor flap is raised mainly from the parietooccipital portion of the scalp. The method can be applied not only to cicatricial alopecia, but to baldness as well.

The number of patients seeking dermolipectomy of the trunk and thighs is increasing among the Japanese. The bodily proportions of the Japanese are different from those of Caucasians. Furthermore, the Japanese tend to hypertrophic scarring. We report our thigh lift technique, including preoperative and postoperative management.

摘要:
A new method for reconstruction in microtia is described. The auricle is divided into two components—conchal and helical. The conchal component includes the conchal cavity, antihelix, superior and inferior antihelical crura, and tragus, while the helical component includes the helix and scapha. In a departure from ordinary methods, the conchal part is reconstructed first and the helical part second. Carving and fabricating costal cartilages, usage of soft tissue flaps, and creation of the tragus are important. Especially important in this technique is conchal restoration. The advantages and disadvantages of the method are also discussed. The author recommends starting the procedure when the patient is about 6, as western surgeons normally do.

摘要:
A surgical technique is described for correcting the drooping alar rim associated with cleft lip. An incision is made on the cleft-side alar rim such that the rims will be symmetrical. After extensive undermining between the dorsal skin and alar cartilages, a suspension suture is positioned so as to pull the drooping point of the nostril rim in an anteromediocranial direction to the septovestibular junction on the normal side. The upper and lateral walls of the nasal vestibule, together with the alar cartilage, are fixed to the dorsal skin in a pushed-back position using a special technique.For the nose to retain its corrected shape, it is important that hematoma does not develop and that the chondrovestibular flap be fixed to the dorsal skin under adequate pressure and over a wide area.

摘要:
Little is known about the cause of scar contracture. The contractile capability of myofibroblasts in the early scar provides a partial explanation; however, clinical and experimental findings indicate that the early scar possesses plasticity, and that its size and shape can be altered by mechanical forces. Even if a scar possesses no contractile capability itself, scar contracture may develop if: (1) the affected joint (skin) is kept in flexion (relaxation); (2) the scar is over a contracted wound; or (3) compression is applied to the early scar. Many instances of scar contracture may therefore be attributable not to active motility but to the passive response of scar tissue to mechanical forces.
