A technique was investigated for possible application in the treatment of myocardial ischemia and for repair of ventricular aneurysms. Six dogs underwent operations to transpose the latissimus dorsi muscle onto the surface of a tangentially excised area of myocardium. Three dogs were reexplored at 4 months, and 1 was reexplored at 1½ years, A firm union of latissimus dorsi to myocardium was demonstrated macroscopically, and there was microangiographic evidence of neovascularization. The latissimus dorsi muscle flap provides considerable bulk, leaves a minimal donor site defect, has the potential for neovascularization, may provide mechanical cardiac assistance, and is simple to transfer. Further investigation is indicated.

Full-thickness chest wall defects after ablative surgery for metastatic cancer, trauma, infection, or irradiation injury have posed major and often impossible dilemmas for reconstruction. At times, resection has had to be abandoned because reconstruction was deemed infeasible. However, recent knowledge of the multicentric blood supply and anatomy of the latissimus dorsi and pectoralis major muscle make it possible to use these muscles and myocutaneous units (even following division of their major blood supply) for reconstruction of the chest wall. The reconstruction of difficult anterolateral chest wall defects is discussed and the options available to the surgeon before and after interruption of the major blood supply to these flaps are presented.

We present a concept in muscle and myocutaneous flap transfer whereby distally based flaps can, under very special circumstances, be transferred with an augmented arc of rotation and more reliable circulation. The concept is the inclusion of adjacent longitudinal vessels in the elevation, within which vessels the flow is reversed. To our knowledge this concept has not been previously described. The soleus muscle and eight other muscles have been found to be potentially useful in this regard. We report here our experience with 3 clinical cases of the distallybased soleus flap with a reversed pedicle, exemplifying the vascular detour principle. Our limited experience indicates no impairment of blood supply to the limb in shortterm follow-up. Until further studies confirm the safety of major vessel sacrifice in a limb, we recommend only judicious use of this flap in highly selected situations.

The importance of resecting a strip of galea in surgery for the elimination of forehead wrinkles is reemphasized. High placement and conservative excision of the frontalis muscle will prevent depression. Elevation of ptotic eyebrows requires a separate procedure. Segmented lift of lower brow is discussed to suit individual need. These procedures are not without pitfalls, but they can be easily prevented by careful planning and execution.

A study was designed to investigate the safety of angiography immediately preceding free flap transfer. Orthotopic abdominal wall free flaps were constructed in uninjected rats, in saline-injected controls, in animals injected with Hypaque, and in animals injected with Conray. No difference in free flap survival could be found among any of these groups. It was concluded that early preoperative angiography does not adversely affect free flap survival.

Medical third-party intervention was a venerable medieval tradition. Fifteenth century London's medical malpractice legislation and court cases contained forms of peer review, compulsory consultation for critical cases, and a malpractice insurance “floater” policy for conditions likely to lead to death, maiming, or accusations of malpractice. A fourpart Latin document from 1415 and a few earlier and later manuscript rules demonstrate the role of politics in medical ethics; civil enforcement of surgical guild regulations; and ingenious forms of protection for patient, practitioner, the surgical profession, and the English citizenry. Pairing compulsory consultation with malpractice insurance policies for high-risk cases offers inspiration for alleviating some modem malpractice perils.

A patient is presented in whom close cooperation between the patient, the general surgeon, and the plastic surgeon allowed cryopreservation of the nipple at the time of initial mastectomy. Clinical and pathological screening was used to ascertain the safety of preservation. Routine two-stage reconstruction was effected, with the exception of the cryopreservation of the nipple-areola graft. Nippleareola grafting was done in two stages to allow transplantation of a more prominent nipple by a technique reported elsewhere [1].
