Cartilage grafting in some projecting nasal tips can enhance the result obtainable by modifying the alar cartilages alone: where there is a sizeable discrepancy between skin sleeve and skeletal volumes, or where a small tip requires a change in configuration. These principles are defined and illustrated in primary rhinoplasty.

Rupture of the silicone prosthesis can occur following external closed compression capsulotomy for fibrous capsules that develop after augmentation mammaplasty. The forces needed to rupture the fibrous capsule can cause silicone gel from the prosthesis to be pressure injected into the breast tissue and surrounding musculature. Film-screen mammography can be utilized to diagnose prosthetic rupture.

We report the results of a prospective study of 45 burn patients in whom serum and urine levels of cream constituents (sulfadiazine and propylene glycol) were determined and correlated with clinical parameters and the presence of an immune response to sulfadiazine. Propylene glycol, measured by gas chromatography, was found in the serum of 53% of study patients. Sulfa, determined by spectroscopy, was found in the serum of all patients, with values as high as 9.1 mg/dl within 24 hours of topical application. Ninety percent of tested patients had circulating sulfadiazine antibodies, predominantly IgG. Fourteen patients died, 11 of whom had interpretable renal biopsies. Five of these biopsies showed positive immunofluorescent linear or granular staining for IgG, IgM, IgA, or C'3; none of these 5 patients had clinical evidence for the nephrotic syndrome. The association between antisulfadiazine antibodies, circulating immune complexes, immunoglobulin disposition on glomerular basement membranes, and topical application of silver sulfadiazine provides evidence for sulfadiazine sensitization in patients with severe burns.

The effect of drug-loaded synthetic dressings on the contraction, epithelialization, and collagen synthesis of deep second-degree burns was followed for 21 days in 44 guinea pigs. Two symmetrical burns were inflicted on the back of each animal: the animals were then divided at random into the following test groups: Group 1 (n=10), a prefabricated, precut, pliable, and noncracking dressing designated as Dimac (DM), versus Dimac plus 2% silver sulfadiazine (DM +); Group 2 (n=10), Hydron loaded with 2% silver sulfadiazine (HAgS) one versus fine mesh gauze and 1% silver-sulfadiazine cream (GAgS); Group 3 (n=12), Hydron (Hyd) versus fine mesh gauze (G); Group 4 (n=4), fine mesh gauze versus Telfa (Tel); Group 5 (n=6), Telfa versus Telfa; Group 6 (n=2), gauze versus gauze. Epithelialization and contraction rates were measured at dressing changes on postburn days 6, 12, 18, and 21, with a computerized sonic digitizer. Collagen biosynthesis was measured from the wound scar on PBD 21 and expressed as relative collagen biosynthesis. Contraction on PBDs 18 to 21 was significantly lower (p<0.05) in the DM and DM+ treated burns. Epithelialization of the DM and DM+ treated groups on PBD 21 was significantly (p<0.05) higher than in the Hyd, HAgS, GAgS, and Tel treated burns, and did not differ from the G treated wounds. The relative collagen biosynthesis method was inapplicable to this burn wound model.

This study investigated the effect of purified bovine collagen (Zyderm collagen implant) on peripheral nerve tissue in rats. Zyderm was injected extrafascicularly and intrafascicularly into the sciatic nerve of adult Wistar rats. The nerves were examined by light and electron microscopy and the integrity of the blood-nerve barrier assessed by protein tracer technique. With light microscopy the precise anatomical location of the injected material could be identified. The purified bovine collagen did not appear to be neurotoxic. After extrafascicular injection, only minimal nerve fiber damage was seen. Intrafascicular injection produced a histological picture compatible with compression neuropathy and was associated with a breakdown in the blood-nerve barrier.

A contralateral transverse rectus abdominis island musculocutaneous flap was employed in the treatment of a unilateral burned breast. The vascularization of the flap was based on the superior epigastric pedicle.The advantages of this method are that a transverse island musculocutaneous flap of the rectus muscle can be used with good results in the reconstruction of the burned breast; and that in patients with hypertrophic contralateral breasts, the inferior pole of the opposite breast can be added to obtain a larger area of the flap. The cutaneous color and the texture offered by this method assure a more symmetrical appearance in the area.

This study demonstrates that both axial and musculocutaneous flaps are similar in their ability to reduce bacterial concentrations and that either is superior to a random flap. Moreover, when the vascular flow in a random flap is improved, so is its potential to diminish bacterial contamination.Clinical infection reflects an imbalance between vascular supply and bacterial contamination. This study implies that flap selection in contaminated areas should be based on vascular profusion and that approaches which preserve or augment blood flow are efficacious in avoiding bacterial proliferation.

Three cases of hand injury caused by meat grinders are presented. All 3 injuries involved the dominant hand and resulted in varying degrees of deformity. Two of the 3 patients arrived in the emergency room with the injured hand still firmly wedged in the meal grinder. Although these injuries continue to prove very mutilating, maximum restoration of the injured hand can be accomplished by careful extrication, followed by preservation and reconstruction of all viable tissues. Perioperative antibiotics and wound irrigation with antibiotic solution are recommended. Microsurgical technique can be of value in treating selected patients.
