The aim of this research was to evaluate the effect of augmentation mammaplasty on women and to determine if it is possible to predict which women will benefit most from the operation.Interviews were conducted with both women who were about to undergo augmentation mammaplasty and those who had undergone the surgery. These interviews were compared with interviews from a control group composed of a normally distributed population.The women who apply for augmentation mammaplasty are a homogeneous group who experience their breast problems in a very similar way. Our study revealed that they are distinctly different from the control group in personality and childhood environment.Most of the women were generally satisfied with the operation even though many had complaints about the technical result. Certain behaviors had changed, but despite these positive results these changes were not radical and in reality the improvement was not as the women initially described it. The women's behavior reactions rarely changed enough to become similar to the reactions of the control group.Using a personality test, it is possible to predict which women will benelit most from the operation. When an assessment of psychiatric problems was made using the neurosis scale in a personality test, the women who scored low on a scale of neurotic self-assertion were satisfied with the operation to a greater extent.

The rabbit fibula was used as the experimental model in this study of the biological changes in nonvascularized and vascularized bone grafts (N= 72). Radiological study showed that the time required for union was about the same for both types of graft. Histological study with tetracycline as a marker showed that there were cellular changes in these two types of graft: moderate hematopoietic and osteocytic loss associated with an increase in bone porosity in vascularized grafts and virtually total osteocytic renewal and marrow necrosis in nonvascularized bone grafts. Quantitative study of vascularization by flow measurement using labeled microspheres showed a brief initial phase of hypervascularization in vascularized grafts, while revascularization took place early in nonvascularized grafts and was followed by hyperemia from the first to the third month. Calcium uptake curves for the two types of graft were quite closely superimposable upon blood flow rate curves. However, the comparative results presented in this study may be tempered by the fact that all grafts were applied to a recipient bed of excellent quality, consisting of well-vascularized muscles.

Genioplasty results in aesthetic and functional changes to the chin and perioral areas. A physiological approach combines osseous and soft tissue reconstruction to produce a superior result that is especially important in individuals with bilabial incompetence. In this review, facial muscle hyperfunction and vertical maxillomandibular excess are identified and their treatment outlined. The modified Michelet genioplasty, an ideal technique for vertical chin reduction and/or advancement, is described.

The histological fate of an injectable form of type I bovine dermal collagen (Zyderm collagen implant) in the human dermis and subcutaneous tissue has been studied. Sequential biopsies were analyzed by hematoxylin-eosin and Weigert's stains, by immunofluorescence (using a highly specific rabbit anti-Zyderm collagen antibody and a specific anti-human type III collagen antibody), and by electron miscroscopy. The results of this study suggest that the bovine implant material stimulates a host response resulting in implant degradation and replacement by newly generated host collagen.

Eleven circumscribed, full-thickness burns were treated in 9 patients with immediate excision and primary closure of the defect or by using a variety of local random cutaneous and musculocutaneous flaps. The burns were located on the face, trunk, and extremities, in 2 patients this technique was used in the management of smaller burns on one surface of the body, thus facilitating skin grafting of larger wounds on the opposite surface. The cutaneous flaps utilized included advancement flaps, V-Y flaps, Moberg flaps, and S-plasties. The timing of the excision and closure varied from 2 hours to 2 weeks following thermal injury, with no postoperative wound infections. In selected cases primary excision of deep burns and closure by use of local tissue approach ideal treatment. With certainty of the depth of destruction, this procedure should be carried out regardless of locality when technically and anatomically feasible.

Although rotation of musculocutaneous flaps can achieve closure of pressure sores, these insensitive flaps in the paraplegic or quadriplegic contribute to recurrent ulceration. This report suggests a method of reinnervating the territory of the lateral femoral cutaneous nerve (the tensor fascia lata musculocutaneous flap) using the medial antebrachial cutaneous nerve of the forearm. This neurotization procedure has restored sensibility to the area of the healed pressure sore.

We present a modified version of the cervical collar that can be used immediately postoperatively and accommodates bulky bolster dressings. Its benefits include immediate neck hyperextension and immobilization and parallel monitoring of a skin graft or free flap.

One of the most frequent complications of facial and frontal rhytidectomy is the formation of hematoma. There are many ways to prevent hematoma formation, but the best one is still absolute hemostasis during surgical procedures. We present our experience with the transnasal route to bring out drains after frontal rhytidectomies. We believe this route is more natural and gravitational and conforms to the general principles and rules concerning drains. We have used this route in more than 120 patients without complications.
