Of the various materials currently available for reconstruction of bony frontal deformities, bone cement (methyl methacrylate) has been judged to be superior in its simplicity, reliability, and aesthetic potential. It is uniquely suited to reconstruction of irregular defects of the forehead. Its biological characteristics, advantages, and hazards are presented along with the techniques of its use. Clinical examples illustrate the results obtained with minimal preparation, surgical time, and morbidity.

Because of the anatomy of the deep lobe of the parotid and its restrictive boundaries, retromandibular tumors may be asymptomatic until reaching massive size. Removal by an intraoral approach may be not only disappointing but also disastrous, with disintegration of the tumor and generalized spillage in the wound. Our experience suggests that the best approach to any type of tumor in this region is through the upper lateral cervical tissues with consideration of a mandibular osteotomy if the tumor extends into the retropharynx or the nasopharynx. This review of 12 patients who required osteotomy describes a high success rate with minimal complications.

Based on our clinical experience, various methods of donor or recipient vascular pedicle coverage for an arterialized or free flap are reported. The vascular pedicle may be placed in the subcutaneous space either by incising the skin along the full length of the vascular route or by passing it through a subcutaneous tunnel. It has been our experience that the vascular pedicle can also be covered by either a local flap or a free skin graft. On rare occasions, a narrow skin flap strip including the vascular pedicle can be lifted en bloc and rotated toward the base of the area transferred.

Closure of large or cosmetically important surgical wounds is often a difficult problem. The utilization of contiguous island flaps—the double V to Y technique—is evaluated as a primary method of closure. The results obtained with this technique were of high cosmetic quality in all areas of the body in which surgery was performed. Use of the technique avoids tissue distortion, cosmetic defects, grafting, and later reconstructive surgery.

The incidence of locally recurrent carcinoma after mastectomy is low—between 6 and 20%. Recent case histories of 4 patients who were clinically free of breast carcinoma and who developed local recurrence after reconstruction are reviewed. Possible relationships of breast reconstruction to local recurrence are discussed. The development of capsule formation in the reconstructed breast may denote recurrent disease.

Subcutaneous mastectomy consists of the removal of the mammary gland while preserving the skin and subcutaneous tissue for reconstruction. The significant amount of tissue in hypertrophic and ptotic breasts makes possible reconstruction by local dermofat flaps. The authors describe a technique for immediate reconstruction which is a combination of the Pitanguy marking technique for reduction mammaplasty and the Maliniac technique for reconstruction using dermofat flaps. The breasts are marked as for reduction mammaplasty; intervention begins with the deepithelialization of the marked area followed by incisions that leave an inferior dermofat flap. Glandular resection results in a cavity bounded by tissue. Reconstruction is carried out by the fixation of the inferior flap to the pectoral fascia, and molding is completed by uniting points at the midclavicular line. This surgical technique has been carried out successfully in 18 patients. Immediately following surgery all symptoms of fibrocystic disease disappeared; areolar sensitivity and erection returned four months after surgery, and aesthetic results were excellent. Breast contour was maintained, and dimensions were proportional. Patients have been followed between ten and twenty-two months postoperatively.

The sciatic nerve in 60 rats was transected and repaired with 9–0 nylon under magnification. The repairs were tested for bursting strength at 2, 3, 6, 9, and 12 weeks and expressed as a percentage of the bursting strength of a paired control from the opposite leg. The mean percent bursting strength was 25, 42, 53, 45, and 62, respectively. There was no statistically significant gain in bursting strength from the third to the ninth week after nerve repair. However, there was a significant difference in bursting strength at 3 weeks and 12 weeks after repair. A similar experiment was performed in monkeys. At 4 weeks, the repaired nerves had regained 77% of the bursting strength of the normal median and ulnar nerves. These studies call into question earlier claims that repaired nerves regain normal bursting strength within 3 to 4 weeks of repair.

A simple and effective method of smoothing glabellar depressions and frown lines is described. Filling the area beneath the frown line or depression with fibromuscular tissue is combined with light, superficial epidermal abrasion and redirection with temporary paralysis of corrugator function, using small and inconspicuous incisions in hidden areas.

The use of an intramedullary stemmed flexible (silicone) hinge in reconstruction of the proximal interphalangeal joint has allowed us to obtain more predictable, durable, and reproducible results. The indications, surgical technique, and postoperative rehabilitation of this technique vary with each type of deformity presented and must be carefully followed.Adequate reconstruction of the extensor mechanism and the collateral ligament system, and appropriate postoperative rehabilitation are essential to obtain good balance of the digit, with functional mobility and stability of the reconstructed proximal interphalangeal joint.
