Information is presented from a clinical study of 65 women and a psychometric study of 85 women having breast reconstruction following mastectomy for breast cancer. The average woman was in her midforties. Less than a third of the women described themselves as fulltime housewives; most were employed outside the home. Almost all had completed high school and most had received additional education. Most of the women were married when they sought breast reconstruction.The time at which women had had their mastectomies was related to their source of information about breast reconstruction. When more than five years had elapsed between mastectomy and reconstruction, a woman most often had obtained her information from the media. Women with more recent mastectomies, though utilizing the media, obtained their information more frequently from medical sources.

Women seeking breast reconstruction did not exhibit characterological problems. Relatively few were in psychiatric treatment, and previous research had established the presence of positive rather than negative self-images.The marriages of women seeking this surgical procedure were rated positively and the women viewed their marriages as a chief source of support. Considerable satisfaction was expressed about husbands being supportive, although husbands were also viewed as poor listeners and, at times, as unable to communicate effectively. Most women were satisfied with various aspects of their sexual lives. A small percentage of the women reported problematic marriages. Various factors, related to the presence of conflict in the marriage and the ability of the husband to understand his wife, may account for the distress evidenced in these marriages. Such factors, of course, are present in any problematic marriage; it remains to be demonstrated whether these problems were caused or exacerbated by the woman's mastectomy and decision to seek breast reconstruction.

An attempt was made to determine if the tissue response to surgical trauma and foreign body stimulus (silicone implants) could be altered using vitamin E in rats. The animals were divided into four groups: Group A served as controls, Group B were treated by intramuscular vitamin E, Group C were treated topically with vitamin E around the prosthesis, and Group D were treated topically with croton oil around the prosthesis. The animals in each group were killed at 2 week, 1 month, and 3 month intervals. The intraprosthetic pressure in each prosthesis was recorded using a strain gauge transducer. The capsules were then removed and examined histologically using a light microscope and the thickness of pseudocapsules was measured with an ocular micrometer.In Group B significantly thinner pseudocapsules were observed at 2 weeks, but there was no comparable difference either in thickness or degree of contracture (as measured by intraprosthetic pressures) between Groups A and B at 2 months and 3 months. In Group C, the pseudocapsules were significantly thicker at all tested periods and showed noticeable contracture at 3 months. In Group D the pseudocapsules were thickest and cellular infiltrate more marked than in the other groups.

A method for constructing groin flaps is presented which the author has used successfully. The flap is elevated medial to the sartorius muscle. The lateral femoral cutaneous nerve is preserved. The muscular branch is carefully divided to avoid damage to the superficial circumflex iliac artery. Near the femoral artery, the important draining vein, which often diverges from the cutaneous artery, must be preserved. One side of the flap is cut longer than the other and diagonal closure of the donor site and the flap are performed. This diagonal closure increases the circumference of the tube at the base of the flap, thereby reducing the risk of vascular compression. Diagonal closure also produces a spiral in the tube. The direction of the spiral can be controlled to facilitate closure of defects on the palm or dorsum of the hand.

A variation of the pectoralis major myocutaneous flap in head and neck reconstruction is presented. The defect to be covered was a large, intraoral, U-shaped defect extending from the left tonsillar region around the alveolar ridge and floor of mouth to the right soft palate and buccal mucosa. Nearly the entire tongue was intact, and a 4 cm left mandibular segmental defect existed. A single left pectoralis major myocutaneous island flap was split longitudinally to resurface the defect. Implications of vertical splitting of a myocutaneous flap are discussed with respect to blood supply and limits.

Absence of the lateral canthal tendon, whether congenital or acquired, imposes cosmetic and functional deformities that are surgically correctable. We propose a means of reconstructing the lateral canthal tendon using a Y graft of palmaris longus tendon. The benefits of this method are discussed. We believe it offers an alternative approach for supporting the lateral canthus and lower eyelid, and is especially useful when reestablishment of the lateral canthal tendon is necessary because of local tissue destruction or congenital absence. Three patients with congenital absence of the lateral canthal tendon are presented.

A new technique for closure of rounded defects is described. The biwinged procedure consists of excising two equal triangles on either side of the defect, one above and one below the dissecting line. Closure is in the form of a step. It can be made to parallel in part a wrinkle or contour line, such as a nasolabial fold, a temporal region above and beside the eyebrow, an area over the lower border of the mandible, or on the dorsum of the hand. A mathematical comparison of the amount of healthy tissue in the biwinged excision and in six other procedures is presented.

The “tumoral” calcifications seen with polymyositis and dermatomyositis are frequently large and painful. Many surgeons believe such calcifications should not be removed surgically, arguing that the surgical trauma induces further calcification that is probably ever more severe. However, we believe that surgical removal of these large masses is less traumatic than cyclical, spontaneous extrusion of the calcium with secondary healing. We present 2 patients in whom chronically draining and painful dystrophic calcifications were treated surgically without recurrence.

Infection is the major cause of morbidity and mortality in burns. Burn wound infection is defined as burn wound bacterial proliferation in a density equal to or greater than 105bacteria per gram of tissue. Gram-negative bacteria, notably Pseudomonas aeruginosa, as well as staphylococci and fungal opportunists, have been identified as prominent invaders.Topical and systemic antimicrobial agents are essential adjuncts in the prevention and treatment of burn wound infection. Topical antimicrobial therapy is indicated in all hospitalized burn patients. Short-term use of systemic antimicrobials for prophylaxis and treatment is required in all moderate and major burns, specifically for early prophylaxis, perioperative prophylaxis, and clinical infection. Antimicrobial choice is based on specific patient or environmental bacteriological data.
