&NA;A new technique for breast reconstruction following mastectomy is suggested. It is based on three principles: (1) use of excess abdominal skin that is stretched upward; (2) use of skin from the healthy breast, when breast reduction or corrective ptosis is necessary; and (3) transposition of skin and excess subcutaneous tissue from the abdomen and the healthy breast, as a contralateral flap (abdominomammary flap), but with a homolateral and a heterolateral pedicle.The abdominomammary flap. adequately deepithelialized, provides material for subclavicular and axillary contouring, as well as a first, partial, or total covering of the prosthesis, while the cutaneous covering is obtained by stretching upward the thoracoabdominal skin of the affected side.

&NA;The pericranial membrane is of great interest in corrective and reconstructive procedures of the craniofacial region. A close look at its ultrastructure reveals that the outer layer is made up of loose areolar tissue and fibroblasts, and the inner layer of osteoblasts. A network of vessels near the inner layer accounts for the ability of the pericranial layer to be used as a flap based on a narrow pedicle.

&NA;We analyzed 122 lateral femoral circumflex arteries, using arteriograms and cadavers, to establish more precisely the course of the dominant vascular pedicle of the tensor fascia lata myocutaneous flap. Measurements were also done for preoperative planning.

&NA;Experience with late escutcheon reconstruction in 4 adult patients, 2 men and 2 women, is presented. All had similar presenting complaints of abnormal external appearance of their pubic region and genitalia, as well as one or more typical related problems. Our surgical approach and correction of these conditions were simple and definitive.

&NA;This is believed to be the first reported case of carotid artery thrombosis after elective maxillary or mandibular osteotomy. The most likely cause was a sudden, sharp blow to the right carotid artery region either when the mandibular split was completed or when the maxillary tuberosity was separated. The blow was associated with neck hyperextension and some lateral flexion. In a patient with tight, underdeveloped soft tissue, the resultant intimal tear caused thrombosis of the artery. Although this event is associated with a 40% mortality and further 52% serious morbidity our patient has regained nearly full neurological function—probably thanks to a competent circle of Willis that allowed adequate collateral flow to the brain.

&NA;Microvascular surgery has greatly altered the handling of scalp avulsions. Review of the injury mechanism and of successful replantations has yielded some basic axioms concerning treatment of this tragic injury. Generally, larger avulsions contain vessels that are easily identified and anastomosed, and therefore have a better prognosis. Long periods of ischemia do not appear to threaten graft viability. The superficial temporal artery is the most reliable artery for scalp replantation; accompanying veins can normally be used for venous drainage, though vein grafts are often needed due to vessel disruption at injury. Replantation should be attempted with all scalp avulsions, though the size of the defect and its location will affect the outcome.

&NA;Surgical reconstruction of the phallus and urethra is normally difficult and time‐consuming. A case is reported using a groin flap for the phallus and tubed, skin‐grafted fascia for the urethra. The new urethra showed little contracture and was easy to construct.

&NA;Ecthyma gangrenosum, the rapidly fulminating tissue infarction ofPseudomonassepsis, presents as one of the more spectacular disease processes. A case involving sloughing of the soft palate, pharyngeal walls, and 40% of one extremity in an infant is presented. Fever and diarrhea are the usual nondescript precursors. The lesions are impressive for their lack of visible inflammatory response or granulation tissue. Early debridement is mandatory for resolution of the toxic condition.
