Modem microsurgery techniques using perineural interfascicular-nerve suturing permit successful nerve reconstruction in tumor defects and injuries of the maxillofacial area. Since 1972 we have concentrated on nerve grafts and reconstructed the facial, accessory, hypoglossal, lingual, and mandibular nerves. In all cases, exclusively autologous nerve grafts were used; as donor nerves we found the great auricular and sural nerves especially suitable. Along with typical examples of the different nerve defects in maxillofacial surgery, our technique in these special nerve transplantation procedures is described.

The two most ideal and versatile foreign materials for reconstruction of facial bone deformities are silicone rubber and methyl methacrylate. Their biomechanical characteristics are uniquely suited to facial implantation for reconstruction of complex, irregular bony defects. The advantages and disadvantages of each are discussed, along with specific indications for their use. Surgical approaches and various methods of fabrication and fixation are presented and illustrated. The hazards and potential disappointments in the use of these and other alloplastic implants are reviewed.

Silastic has been used with varying success as an implant material. When Silastic implants fail, migration and infection are generally involved. Perforation of the implant has been suggested as a means of minimizing implant migration. In this study, 30 implants were studied in Rhesus monkeys. The implants varied in external geometric form and in perforation size.Results indicated that fibrous encapsulation of the implant occurred within two weeks. The tissue readily invaded the perforations regardless of pore size. The configuration of connective tissue was unaffected by external geometry. Perforated implants were less mobile than nonperforated implants, and implants with perforations of 3 mm or more contained dense fibrous connective tissue supported by an underlying vascular mesenchymal tissue not seen in implants with smaller pores. As a result of this study, we recommend that perforations be used in Silastic implants and that these perforations be at least 3 mm in diameter.

A standardized technique for fabrication of customized perforated medical-grade room temperature-vulcanizing silicone rubber implants for tissue augmentation is described. This method produces an implant that has proved successful in over fifteen years of use in our institution.

Patients with severe mandibuiar piognathism are best managed with a combined orthodoiitic-suigicdl approach. In our patients, the orthodontic treatment consisted of six to eighteen months of presurgical preparation, which in some patients may accentuate the dental deformity. This is done to provide two well-aligned dental arches that will fit accurately at surgery. The surgical procedure used was an oblique subcondylar osteotomy. This was followed by six to eight months of orthodontic treatment to complete dental alignment. Thirty patients were treated using this combined approach, with excellent results and few complications.

The author discusses the various problems that can arise in the static or dynamic state in the eyeball, the eyelids, and the lacrymal canals—due to alterations of the bony orbit. Reconstruction techniques in the orbitomaxillomalar, orbitonasal, and frontoorbital regions are dealt with separately, with emphasis on the need for a global and primary solution to all the alterations.Particular attention is payed to some cases of fibrous dysplasia in the frontoorbital region that were treated with reimplantation of the same dysplasic bone after it was remodeled. This is a personal technique that is suggested as an alternative treatment for this kind of lesion.

Necrotizing sialometaplasia is a benign self-limited lesion usually affecting the salivary glands and mucosa of the palate. It is easily confused both clinically and microscopically with malignancies for which radical surgery or radiation therapy may be indicated. A high index of suspicion and knowledgeable micropathological interpretation of adequate biopsy specimens can, however, help prevent therapeutic misadventure.
