Cranioplasties of the frontoorbital area pose a major challenge to the surgeon because of the proximity to the globe, the sinuses, and the eyebrows and eyelids. The choice of material for reconstruction includes autogenous and alloplastic materials. A modification of the fixation technique of split rib graft to avoid depression deformities at the periphery of the defect is described. Sixteen patients were reconstructed with different treatment modalities, including the use of alloplastic materials or autogenous bone or a combination of both. The results are reported as are the complications. Limitations encountered with these difficult reconstructions are discussed.

Low-voltage electrical burns were studied in 25 pigs submitted to bilateral thigh burns using 500 volts AC for 10 seconds. In a group of 6 animals, the wounds were observed and histologic specimens obtained at different intervals during the first week. The burn measured approximately 4 cm in diameter and extended through two muscle layers. This tissue underwent further necrosis during the first 48 hours. After 2 days there was no further necrosis. In a second group, the wounds were debrided and covered with either a split-thickness skin graft or a rectus abdominis musculocutaneous flap. All wounds with flap coverage healed primarily and there was no deep necrosis. From our findings we conclude that debridement and definitive closure can safely be performed 2 days after low-voltage electrical burns.

This experimental study in the rabbit establishes a reliable model for the production of skin necrosis analogous to intravenous extravasation injuries in humans. The effectiveness of hyaluronidase in reducing the extent of tissue injury is examined, using several different toxic agents.

Otoplasty should restructure the cartilage framework of the ear with permanent results. Major techniques include the placement of permanent sutures, with or without concomitant cartilage scoring, and cartilage abrasion alone. Our study evaluates the durability of the results of each method.Fifteen New Zealand white rabbits were divided into two groups. In 10 rabbits, the posterior surface of the right ear cartilage was abraded, and the ear was folded forward and secured with nylon stitches. A similar procedure was performed on the left ear, omitting the cartilage abrasion. In 4 rabbits, both ears were abraded transversely on the posterior cartilaginous surface and no sutures were placed. In 1 rabbit only the posterior skin was undermined on both ears. For one week dressings maintained a right-angle fold at the operative site.Rabbit ears reshaped with abrasion only failed to maintain the desired conformation. Ears with suture and abrasion did not maintain the full 90-degree angle created at surgery, although they did not fully straighten. Ears reshaped with suture alone maintained a folded angle close to the original right angle. Three months after operation the sutures within the cartilage were removed to approximate the clinical situation of late suture disruption from trauma to the operated ear and the shape was assessed.

Twenty-one men with genital Paget’s disease were treated in our clinic from 1968 to 1983. We report our policy regarding treatment of the clinical disorder, particularly our surgical approach. We also stress that, when considering treatment, the circumstances of the aged patients should lake precedence over aggressive treatment of the clinical state of the dermal disease.

Giant cell tumor of tendon sheath (localized nodular tenosynovitis) remains a disease of unknown origin. Thirty-five giant cell tumors of tendon sheath are reviewed in this study. High recurrence rates can be minimized with attention to accurate preoperative diagnosis and aggressive local excision. The removal of portions of tendon sheath, volar plate, periosteum, or joint capsule may be necessary in selected cases.

Two methods for measuring area, the sonic digitizer and the polar-planimeter, were compared for speed, accuracy, and efficiency. The sonic digitizer was significantly (p < 0.001) faster than the planimeter (97.2 ± 9.5 versus 206.3 ± 14.8 seconds per flap) and more efficient. The accuracy of both methods in measuring areas of 5 to 30 cm2was comparable; however, for areas of 5 to 13 cm2the sonic digitizer gave more reproducible results (p < 0.001). These differences were not reflected in the computed rate of necrosis of the present flap model.

Because injudicious surgery may adversely alter nasal function, those who treat problems of the nose should be aware of nasal physiology. The nose has seven functions: airway, air conditioning, air cleaning, olfaction, phonation, reflex, and secondary sex organ. These are reviewed in detail and their significance to the planning and performance of an aesthetic rhinoplasty, septal surgery, and turbinate surgery appropriately noted. Form and function are related.

Facio-genito-popliteal syndrome, or popliteal pterygium syndrome, is a very rare anomaly consisting of various combinations of facial deformities (cleft lip/palate, lip pits, syngnathia), popliteal pterygia, and genital deformities. We have found 56 cases previously recorded in the literature.

A severe case is reported of a rare skin disorder characterized by multiple intracutaneous cystic lesions; the condition is difficult to manage and impossible to cure. The complicating cystic infections that accompany the condition are treated acutely using sound surgical principles, yet long-term management remains a difficult and unsolved problem. Some of the approaches to treatment are discussed. Surgical cures are usually precluded by the extensive nature of the disease. Perhaps most disheartening is the devastating social and psychological damage resulting from the progressive facial and bodily disfigurement, for which there is at present no satisfactory treatment.
