Thirteen patients with capillary/cavernous hemangiomas of infancy have been treated with a combination of neodymium: YAG laser photocoagulation plus direct local instillation of steroids. In all cases, rapid growth immediately ceased and blanching started. Shrinkage of the lesions then occurred spontaneously over the ensuing months; any plateau in shrinkage was retreated. Five of 13 patients experienced dramatic blanching and shrinkage with one treatment. Six experienced mild to moderate shrinkage. Complications occurred in 2 patients.

Small, palpable, and visible dorsal irregularities may occur in as many as 5 to 10% of rhinoplasties. They can be unacceptable to both the surgeon and patient and can lead to unwanted secondary surgery. In 88 patients treated over an 18-month period, Vicryl mesh implants consisting of one to three layers of Vicryl were placed under direct vision over the dorsal cartilage; care was taken to avoid disturbing any cartilage implants. Early results were impressive; there was an absence of palpable dorsal irregularities. This report describes the surgical technique and impressions during the 18-month period.

Clinical measures often fail to detect early tissue ischemia in inadequately perfused flaps. This study investigates the application of a new pH electrode system to monitor tissue metabolism continuously in porcine and human musculocutaneous flaps. Bilateral rectus abdominis flaps based on the deep inferior epigastric pedicle were elevated in eight mixed-breed pigs. Tissue pH was measured in the subcutaneous and muscular layers with miniature glass-tip electrodes. The deep inferior epigastric artery and vein were serially and then concomitantly occluded for 20-minute periods. The decrease in tissue pH was greater following either arterial or pedicle occlusion (each 0.21 ± 0.03 units) when compared with venous occlusion (0.10 ± 0.02 units; p < 0.02). Changes in muscle and subcutaneous hydrogen ion concentration were similar during arterial and venous occlusions (probability not significant). Measurement of subcutaneous pH appears to be a reliable experimental and clinical physiological tool for detecting early tissue ischemia in reconstructive flaps.

The vast array of free tissue options available to reconstructive surgeons plus our knowledge of vascular systems now provide us with the wherewithal to not only fill a defect but to do so aesthetically and with 'minimal donor site morbidity. The authors discuss setting reconstructive goals that include refined and aesthetic results for deficit and donor site alike and demonstrate how, in many situations, these goals can be achieved.

We have undertaken a microanatomical study of a new donor site–the posterior forearm territory–on 43 cadaver arms. Two axial arteries were found, and both the fasciocutaneous flap and the fascial flap could be elevated with the two arteries on the posterior forearm. The posterior forearm flap was transferred to the hand to repair deformities of the wrist, the first web space, the dorsal hand, and the metacarpophalangeal joints in eight cases. The result was very satisfactory. The posterior forearm flap is near the hand, easy to perform, and reliable. It is a one-stage operation and does not sacrifice a main artery. In this article we demonstrate our own ideas about flap extension, which is valuable for clinical application.

Seventeen forelimbs were transplanted orthotopically from young Dutch rabbits to young New Zealand rabbits treated with cyclosporine. The transplanted limbs demonstrated significant bone growth. The growth in the transplanted limbs was about 75 to 80% of that observed in the un-operated limb. The long bones of the 3 longest surviving rabbits (133 days, 150 days, 150 days) studied radiographically demonstrated increases in length over their original lengths (humerus 22%, ulna 26%, and radius 31%). Hair and nail growth were noted at about day 10. Response to pain stimuli (withdrawal of forelimb) and functional use (ambulation with 50% weight bearing) was seen at two to three months. Permanent survival was not achieved because of a species-specific toxic wasting syndrome from cyclosporine.

Seventy-one patients with facial palsy following implantation of a gold prosthesis into the upper eyelid are reviewed; the average follow-up is 3.2 years. Eighty percent (57) of the patients demonstrated no complications and had satisfactory results. The methods, complications, advantages, and disadvantages are discussed.

Peripheral intravenous fluid extravasation is a common occurrence among neonatal intensive care unit patients. Fifteen high-risk neonates, averaging less than 35 weeks' gestation and less than 1,500 g birth weight, with full-thickness extravasation injuries were successfully treated nonoperatively by a topical fibrinolysin/deoxyribonuclease ointment. All wounds healed without delaying hospital discharge, and no significant scar contractures -were observed in patients followed up to 16 months after injury.
