Total Quality Management (TQM) is a continual improvement process that requires common sense, education and training, and the ability to communicate and work as part of a team. TQM can improve all aspects of a business, including health, safety, and environmental functions. Physicians and nonphysician managers can use TQM to identify and respond to customer needs and improve the efficiency and effectiveness of programs. Examples of TQM to improve medical programs have included (1) an Audit Program that assesses medical programs and provides specific measurements (“metrics”) of medical programs, and (2) a team that developed an Alternative Return-to-Work Program, which now assists in early rehabilitation of injured employees. In addition to expecting its physicians to be knowledgeable, fair, and objective, management expects them to use and understand TQM principles.

Instilling the principles of total quality management (TQM) into organizations is a difficult undertaking, yet many leaders of health care organizations are forging this paradigmatic shift with encouraging results. In this paper, a practical approach is described that has been deployed within an occupational and environmental health setting. The vehicle for this transformation is called the Strategic Quality Plan. Once the purpose was clearly elucidated by the leadership, a basic design of the plan was developed that melded two interrelated goals: achieving strategic business objectives by applying quality principles. The resulting matrix became the basis for detailed flowcharts that show the steps, inputs, decision points, deadlines, and continuous improvement loops for each strategic objective. A deployment plan was developed to aid implementation.

The establishment of medical facilities and a proactive care system for expatriates in a foreign country should be approached in stepwise fashion. In advance of a new company operation, considerable information must be gathered about the company's plan. Risk assessment includes consideration of both occupational and country-related medical issues. Resource assessment generally requires an on-site visit. Establishing the medical plan for a site generally requires the selection of a company doctor and the best available hospitals and local specialists, and upgrading local facilities and site-specific plans. Comparable occupational health programs must be developed for the nationals employed in the country. These and other considerations will prepare the occupational physician to assist for each new company operation, for each new expatriate assignment.

The author was asked to share his experiences in developing medical programs within a company with numerous small manufacturing locations. Successful programs at these locations depend on obtaining high-quality occupational medical services in the local community. Policies that drive the content and quality of medical program services within large companies are described. Factors that are important in selecting and retaining a local provider are discussed in detail as is the ongoing role of the company's medical director in local site medical programs. The importance of program reviews or audits is stressed with information presented on what a private clinic or practitioner should expect during an audit. Impartiality, objectivity, and fairness were emphasized along with the need for good office management and interpersonal skills as characteristics that can help lead to long-term business relationships between corporate clients and occupational medicine physicians in private practice.

A strong regional program can be successful in either a single location or at multiple locations, but it cannot function properly if it is not well organized and staffed properly to deliver services. Extremely important elements to the success of an occupational health clinic that serves a wide geographical region are client retention, a strong information system to manage programs for multiple employers, and cutting-edge leadership that stays in the forefront and educates the business community on emerging issues in occupational health and safety.
