A cohort study of agricultural practices and the mortality experience (1971 to 1987) of male farmers in Manitoba, Saskatchewan, and Alberta has been conducted. The census records of 156, 242 male farmers identified on the 1971 Census of Agriculture and the corresponding Census of Population were linked to mortality records through 1987. We observed associations between multiple myeloma mortality and fuel/oil expenditures (relative risk for top quartile of exposure = 1.69, 95% confidence interval = 1.08 to 2.65). No significant associations were found between multiple myeloma and herbicide use or with selected sociodemographic variables.

Occupational health practitioners have striven for 60 years to develop and implement appropriate accreditation policies and procedures to promote quality assurance of occupational health practice. In 1987 the American Occupational Medical Association affiliated with the Accreditation Association for Ambulatory Health Care, an independent ambulatory health care accreditation organization, to advance this movement. This paper explores the history of occupational health care accreditation and identifies potential incentives needed to encourage accreditation of occupational health care. The Occupational Safety and Health Administration, industry, labor, insurance carriers, and professional organizations collectively must develop strategies to promote accreditation and ensure that workers receive the maximum standard of occupational health care.

Using audiometric data, this study assessed the Draft American National Standard ANSI S12.13–1991 (DANS) method for evaluating the effectiveness of hearing conservation programs by comparing it against a standard epi-demiologic method. Although the hearing conservation program of the study population was rated as “acceptable” (scale: acceptable, marginal, unacceptable) using the DANS method, the epidemiologic method found a 130% increased risk of hearing loss (relative risk = 2.3, 95% confidence interval = 0.8 to 6.5). In addition, study workers who were excluded from analyses for failing to comply with the DANS criteria were found to be at significantly increased risk of hearing loss (relative risk = 9.1, 95% confidence interval = 3.4 to 24.2). These data indicate that the DANS method may overestimate the effectiveness of hearing conservation programs and systematically exclude workers at high risk of hearing loss from analyses.

As part of our longitudinal study of unexposed workers, we drew blood samples and analyzed the individual endocrine profiles for 45 men. The blood collection was between 8 AM and 8 PM, and three blood samples were drawn 20 minutes apart on three occasions during the course of the study (June, October, and February). Serum concentrations of follicle-stimulating hormone, Iuteinizing hormone, testosterone, and prolactin were determined. A component of variance model was used to estimate variability between the 20-minute blood draws. Statistical power analysis using this component showed that three blood draws provide a marginal improvement over a single blood draw in detecting population shifts. Also, if the prospect of three blood draws reduces subject participation by 10 to 20%, the increase in power would be negated.
