This report describes the health effects of occupational exposure during manufacturing processes that utilize synthetic estrogens and progestogens, and demonstrates the usefulness of various laboratory assays in identifying workers who have experienced actual hormone absorption. In late 1978 the health status of exposed and unexposed workers was compared by standardized questionnaire, physical examination, and laboratory studies. A higher proportion of exposed than unexposed men gave histories of breast swelling (p < .0001), nodules (p = .0003), and nipple discharge (p = .037). Physical examination showed no significant differences between groups. Exposed and unexposed workers showed no clinically significant differences on any routine laboratory studies. Hormone studies revealed significantly lower natural total serum estrogen levels in exposed men and women. Serum norethlndrone levels were useful in identifying workers with hormone exposure in their recent past. Hormone exposure continued to cause physiologic disturbances during the period studied despite efforts to control exposure. Measuring blood norethlndrone levels may be a valuable way of identifying exposed workers.

To determine if parasitic infections occur more frequently among workers exposed to wastewater than among controls, parasitologic examinations were performed on stool specimens collected over a 12-month period from sewer and highway maintenance workers. Three of 56 sewer maintenance workers (5.4%) had at least one positive specimen compared with 10 of 69 highway workers (14.5%). The highest rate was found among highway workers engaged in street cleaning. Contrary to studies conducted outside the United States, the present study does not indicate that parasitic infections occur more frequently among sewer workers than among controls in the urban midwestern United States. The results of this study indicate that there may be a risk of parasitic infections among street cleaners.

摘要:
Through an agreement with the Health and Safety Department of the United Steelworkers of America, a faculty member of the University of Pittsburgh's School of Medicine has, for the last five years, devoted 50% time effort to providing medical consultation to the union. The responsibilities of the position are described, and the experience of five years is evaluated. A suggested Code of Workplace Practices is offered to provide some guidance to those physicians involved in providing medical consultation on a part-time basis.

Marine engineers undergoing routine annual chest roentgenography showed an unusual prevalence of pleural abnormalities including plaques suggestive of past asbestos exposure. A pilot survey, and a subsequent comprehensive study of the films of more than 5,000 men, showed an overall prevalence of 12% with pleural abnormality (typical calcification or plaque, or diffuse thickening). Prevalence of films classifiable for pneumoconiotic small opacities was negligible – 1.2% in the pilot study. Prevalences of pleural abnormality were significantly higher among men with longer union membership, after controlling for age. Older merchant ships contain substantial amounts of asbestos-containing thermal insulation. Marine engineers often remove and reapply insulation, operations known to produce high airborne fiber concentrations. These roentgenographic survey results indicate significant past asbestos exposures of ships' engineering department personnel.
