J Oral Pathol 1987: 16: 383–388.The clinical, radiographical, biochemical, microscopical and ultrastructural features of 10 cases of multiple myeloma were studied. The skull and jaw regions were frequently involved by the disease and although the diagnosis remains multidisciplinary, microscopical parameters differentiating a myelomatous bone marrow infiltrate from a reactive plasmacytosis are discussed. Biochemical and microscopical factors influencing the prognosis are highlighted and significant ultrastructural findings include erythrophagocytosis, cytoplasmic nuclear asynchrony of plasmablasts and intranuclear viral like inclusion

J Oral Pathol 1987: 16: 389–391.Quantitation of circulating immune complexes (CIC) levels was established in patients with oral cancer and oral precancerous lesions. The levels were compared with that in normal controls and chronic chewers of betal quid with no signs of any disease. Both patients with oral cancer and oral precancerous lesions had elevated CIC when compared to both the control groups. The most interesting observations were (a) the CIC levels in the chewing controls were significantly raised when compared to normal controls; and (b) the CIC levels in the patients with premalignant lesions were elevated almost to the same levels as in the oral cancer patient

J Oral Pathol 1987: 16: 392–394.Some people exhibit an exceptional tendency to develop brownish extrinsic staining on their teeth and previous studies indicate that iron may be involved in certain types. The object of this investigation was to find out whether the level of salivary lactoferrin, which is an ironbinding glycoprotein, is elevated in persons exhibiting extreme staining tendency. Subjects who developed dark brownish discoloration on the facial surfaces of their anterior teeth during a 3‐week period following professional cleaning of the teeth were selected for study. Salivary lactoferrin was measured by the enzyme‐linked immunosorbent assay (ELISA). The results showed that this group of persons exhibited a markedly higher concentration of salivary lactoferrin compared with non‐stainers. J Oral Pathol 1987: 16: 395–398.Mechanical load influences the remodelling of skeletal tissues. In the mandibular condyle, occlusal alterations and the consequent mechanical stimulus induce changes in chondrocytes and cartilage mineralization. In the present study we quantified in the mandibular condyle the effect of occlusal interference on remodelling of the subchondral bone. Computerized histomorphometry after 5–21‐day exposure to the influence of a unilateral occlusal splint revealed an increased rate of trabecular remodelling, consisting of enhancement in osteoblast and osteoclast numbers and activities. The bone formation parameters reached their high values on Days 5 or 9 and remained stable thereafter. Bone resorption showed a gradual increase throughout the experimental period. These results further characterize the temporomandibular joint reaction to occlusal alterations. J Oral Pathol 1987: 16: 399–402.The deleterious effect on enamel of continual acid challenge from regurgitation of gastric contents in bulimic subjects is well documented. No previous reports, however, have been published on chemical and physical measurements of teeth from such subjects, or on the importance of fluoride in maintaining their dentition. Teeth were obtained from a deceased anorexic and bulimic patient. Shortly before death this patient had saliva analyses andin vivoenamel biopsies at our clinic, before and after a 3‐week regimen of daily rinsing with 0.05% NaF. After 4 years of daily regurgitation there was an almost normal thickness of the enamel surfaces typically “at risk”, i.e. the palatal surfaces of the maxillary anterior teeth, with normal hardness measurements 10μm away from the outer surface. SEM micrographs showed an irregular topography, with crystalline deposits rich in calcium, phosphate and fluoride. These surfaces were more reactive to uptake of additional fluoride when given as a daily rinse, than the more protected, and, therefore, presumed unaffected surfaces. Our findings suggest that frequent and meticulous oral hygiene measures can substantially minimize the erosive effect on tooth enamel of gastric contents in bulimic patients, especially if fluoride products are used

Histologically, 3 of zones characteristic rheumatoid nodules were recognized: a central necrotic area, an intermediate zone with palisading mesenchymal cells and an outer layer composed of chronic inflammatory cells. Paul KG. Peroxidases: past and present. J Oral Pathol 1987: 16: 409–411.The history of peroxidases spans nearly two centuries. Our knowledge has developed from early phenomenological observations of the colored products of peroxidase‐catalyzed reactions, to our present understanding of many of the steps in the complex peroxidation reaction mechanism. Peroxidases are ubiquitous in plant and animal tissues and occur in diverse structural forms. Collectively, they are able to catalyze the hydroperoxide oxidation of many different kinds of organic and inorganic compounds. In spite of the great diversity of structures and functions, mechanisms of heme‐containing peroxidases have several common features: (i) the transfer of the oxidizing equivalents of the hydroperoxidase to the enzyme to form Compound I, (ii) the reduction of Compound I by the transfer of electrons from donor molecules, (iii) the inactivation of Compound I by excess hydroperoxide. Rate constants for these and other steps in the peroxidation mechanism, as well as redox potentials, have been reported for many peroxidases. J Oral Pathol 1987: 16: 412–416.All living cells convert dioxygen (O2) into the toxic intermediates of oxygen reduction: superoxide radicals, hydrogen peroxide and hydroxyl radicals. The defense against these intermediates works on three levels: 1) the formation of the intermediates is prevented; 2) the intermediates are scavenged; 3) the damaged sites are repaired. Various parts of this defense are found intracellular^, in tissue fluid, in blood plasma and in the external secretions. In the mouth there is a special need for defense against hydrogen peroxide, because hydrogen peroxide is not only formed by bacteria colonizing the mucous membranes but also by the cells of the salivary glands. In saliva the most important part of this defense is salivary peroxidase, which detoxifies hydrogen peroxide in the presence of thiocyanate by converting it into hypothiocyanite, dioxygen and water. J Oral Pathol 1987: 16: 417–420.Enzymes are specific catalysts which optimize the rates of reactions vital to living organisms. If the oxidation of thiocyanate (which occurs,in vivo, in human saliva and salivary glands) is a vital reaction for the maintenance of oral health, then the enzyme (salivary peroxidase), which catalyzes this reaction, should function optimally underin vivoconditions. Studies have shown that salivary peroxidase maintains the SCN‐oxidation reaction in an apparent state of dynamic equilibrium, that the kinetic properties of the enzyme provide optimum rates of reaction underin vivoconditions, and that the antibacterial properties of the products of the reaction are optimum under those conditions where unlimited bacterial metabolism provides the greatest threat to host tissues.
