Tobacco and alcohol consumption are known predisposing factors to the development of oral cancer, though the mechanism by which they act is unclear. The density of Langerhans cells (LC) has been shown to decrease in the uterine cervix of smokers, possibly indicating reduced immune surveillance and increased risk of malignant change. This study examined biopsies of normal human lateral border of tongue taken from 41 necropsies whose histories of tobacco and alcohol consumption were known. LC were identified in fixed, wax‐embedded sections using a monoclonal antibody to HLADR and an immunoperoxidase technique. The mean density of LC in smokers of the equivalent of 11 or more cigarettes daily was significantly higher than moderate and non‐smokers when counts were expressed per mm epithelial surface and basement membrane length. There were no significant differences in LC numbers in relation to alcohol consumption, age or sex, but there was a significant interaction between tobacco and alcohol. The results suggest that either human oral LC respond to external toxins, or that physical changes in the epithelial barrier induced by such toxins necessitate the presence of more LC to maintain the integrity of the epithel

Silver‐binding nucleolar organizer regions (AgNORs) were counted in sections from formalin‐fixed, paraffin‐embedded tissue blocks of oral squamous cell carcinoma. Thirty‐nine cases, that comprised poor prognostic group (n= 19) and good prognostic group (n= 20), were examined with respect to the relation between AgNOR counts and histologic grading, and correlation between AgNOR counts and prognosis. The pooled mean AgNOR counts were: Grade 1 carcinomas, 6.39 ±1.67 (mean ± SD;n= 35); Grade 2, 9.74 ± 1.72 (n= 3). Mean AgNOR count of Grade 3 was 6.19 ± 2.37 (n= 1). The pooled mean AgNOR count in poor prognostic group was higher than that in good prognostic group. Five‐year survival rate of the cases with high AgNOR counts (≥ 6.5) was significantly lower than that with low AgNOR counts (<6.5). MAM‐3 and MAM‐6 antigens were detected immunohistochemically in 34 cases of adenoid cystic carcinomas (ACC) of the salivary glands and these patterns were compared to these of epithelial membrane antigen (EM A) and laminin. ACC was histologically divided into three types; the cribriform pattern, the tubular and trabecular pattern, and the solid cluster pattern. Immunostaining of EMA and MAM‐6 antigen had a similar distributions in the luminal borders of luminal tumor cells, whereas the MAM‐3 antigen was slight or negative in luminal borders. Myoepithelial derived tumor cells of ACC accompanying hyaline stroma demonstrated positive staining for the MAM‐6 antigen (whole cell positive type), and luminal tumor cells of microcysts showed strong staining for the MAM‐3 antigen. Laminin staining was confined to the basement membrane and surface borders in pseudocyst cavities. In salivary gland ACC, laminin staining can be used as a marker of pseudocyst surfaces and immunostaining of EMA and the MAM‐6 antigen as a marker of luminal borders of cyst. A prospective study was undertaken of 22 patients with chronic hyperplastic candidosis and 50 age and sex matched individuals with no evidence of oral mucosal disease. Specifically the relationship between the inherited ability to secrete blood group antigens in saliva and chronic hyperplastic candidosis was investigated. To examine the sensitivity of human oral mucosa to herpes simplex virus type 1 (HSV‐1) and type 2 (HSV‐2) infection, human gingival mucosa explants were infected with either HSV‐1 or HSV‐2 in vitro and the expression of virus specific antigen was examined by the immunofluorescent antibody technique. HSV‐2 antigen was found in the basement membrane, basal cell layer and lower prickle cell layer. This finding was consistent with the HSV‐1 infection. Electron microscopic study revealed the presence of nucleocapsids and enveloped virus particles in the basal cells of HSV‐2‐infected organ cultures. These findings indicate that human gingival mucosa is sensitive to infection with HSV‐2, as well as HSV‐1, and that the virus may replicate in the undifferentiated epithelial cells of

A prominent cellular event in tooth eruption is the influx of mononuclear cells (monocytes?) into the dental follicle at the onset of eruption. In the mandibular first molar of the rat, this influx of cells reaches its peak at three days postnatally. Because transforming growth factor‐beta 1 (TGF‐β1) is chemotactic for monocytes, we used immunocytochemical methods to determine its localization in the rat molar during postnatal days 1–4 and day 7. The results indicate that TGF‐βl displays both a spatial and temporal localization in the rat molar. It is present in the stellate reticulum (SR) on days 1 and 2 postnatally but is absent in all the subsequent days examined. None of the other soft tissue layers‐ameoblasts, stratum intermedium or dental follicle – immunostain for TGF‐βl. This localization of TGF‐β1 in the SR at a time that just precedes that influx of monocytes into the dental follicle, coupled with the fact that fenestrated capillaries are abundant in the follicle adjacent to the SR, suggests that TGF‐βl could play a role in attracting monocytes from the peripheral b

In the present study, quantification of different serum proteins known as inflammatory reactans was performed during different stages of chronic osteomyelitis of the jaws, to find a suitable tool for evaluation of treatment. In all 46 sera from 17 osteomyelitis patients and 6 healthy subjects were analyzed. Repeated measurements of alfa‐1‐antitrypsin, orosomucoid and haptoglobin could be recommended for following‐tip the effect of treatment, although too extensive conclusions should not be drawn from single measurements. The serum levels of these proteins seemed to co‐vary with the clinical activity of the disease. Since the synthesis of the immunoglobulins only indirectly reflects the inflammatory activity, they are not considered to be suitable markers of inflammation, although their concentration in serum varied with the clinical activity. This paper reports, for the first time, gingival hyperplasia in a patient treated with felodipine, a drug which belongs to the group of calcium ion antagonists. The observed gingival overgrowth was most significant in the area of interdental papillae of the anterior region of the mouth. The described hyperplastic tissue was characterized by a firm and pale appearance, with a normally stippled pattern. Histopathologically, a conspicuous increase of fibrous connective tissue, as well as an inflammatory infiltrate and hyperplasia of the overlying epithelium were observed. Consequently, the present observation adds another drug to the list of substances capable to induce gingival hyperplasia.

A case of calcifying epithelial odontogenic tumor (CEOT) occurring in the left mandible of a 50‐yr‐old man was characterized by the presence of large masses of calcified collagen. Calcified amyloid acted as the nucleus for the deposition of this material. Because of the close association between both types of calcified material, it is presumed that the calcified amyloid is an inductive stimulus for the stromal cells to differentiate towards production of a collagenous matrix destined to calcify and resembling cementum.
