摘要:
Human whole saliva protects the oral environment in many different ways from invading pathogenic microorganisms. Human saliva is also capable of inactivating mutagenic and carcinogenic agents by various mechanisms. The peroxidation of these agents is likely to be one of the degrading reactions. However, under certain circumstances some potentially carcinogenic compounds, such as N‐nitrosamines, may be generated in whole saliva or ‐even more likely ‐ in the saliva‐gastric juice mixture after swallowing. The formation of N‐nitroso compounds requires relatively high intake of nitrate e.g. from vegetable juices. Nitrate is partly reduced to nitrite by oral bacterial enzymes. The nitrosation of various secondary amines is favoured by high salivary (or gastric) concentration of thiocyanate and by low pH. Twenty‐four cases of adenoid cystic carcinoma from the Salivary Gland Registry of the University of Hamburg were analysed with respect to their differentiation and subclassification. With immunohistochemical methods, several groups of marker substances were analysed in these tumors: marker for a glandular function; immunoglobulins; metalloproteins; tumor markers; substances associated with the basal membrane and carbohydrate chains. The following patterns of histological differentiation were found: cribriform pattern with typical pseudocysts; tubular pattern with ductal elements; trabecular pattern with predominance of hyalininzed stromal elements; basaloid pattern with low differentiated cells. For every pattern, a special immunohistochemical profile could be established. The morphological and immunohistochemical analysis revealed the following relation between the different patterns: Origin of the malignant proliferation is presumably the terminal ductal system of the salivary glands (indifferent cell of the reserve cell type). This cell can differentiate in 2 directions: a myoepithelial and a ductal cell type. Immunohistochemically, the presence of basement membrane‐associated substances is accompanied by the appearance of myoepithelial‐like cells. The presence of secretory products is a feature of ductal

A case of granular cell ameloblastoma with numerous intracytoplasmic crystalloids is reported. The intracytoplasmic crystalloids are linear in shape and up to 15 μm in length, and stained similar to the intracytoplasmic granules of granular cell ameloblostomas which have been identified histochemically as lysosomes. The intracytoplasmic crystalloids in the granular cell ameloblastoma presented here probably reflect a variant type of lysosome, possibly due to cellular degeneration

An unusual giant‐cell granulomatous inflammatory oral lesion is characterised by the presence of hyaline rings and by a lack of agreement on their nature, opinions ranging from their being hyaline degenerative blood‐vessels to remains of leguminous cells. Ten such lesions and a variety of vegetables have been examined in the present investigation. Light‐microscopical examination of the lesions revealed the hyaline rings surrounded by inflamed granulation tissue with associated multinuclear giant cells. Electron‐microscopical examination showed them to consist of a material similar to that of vegetable cell walls, sometimes with an associated superficial layer of collagen. Histological, histometric and ultrastructural studies of rat submandibular salivary glands (SMG) following single amputation of lower incisor teeth were made. The SMG enlargement occurred after the tooth amputation, and its maximal increase of weight was reached 7 days later. The results of histological and histometric observations showed that the enlargement of SMG was due to both hypertrophy and hyperplasia of the acinar cells, and hypertrophic acinar cells were ultrastructurally characterized by well‐developed organelles such as rough‐surfaced endoplasmic reticulum and Golgi apparatus which filled the entire cytoplasm. Hydropically swollen nerve endings with disappearance of vesicles were frequently found. In later stages, degenerative or necrotic cells were scattered in the acini, and the size and the structure of the remaining acinar cells became gradually similar to those of non‐treated glands. The morphological changes seen in the enlarged SMG induced by single amputation of the lower incisor teeth in rat were similar in many ways to those of sialadenosis induced experimentally by active sympathomimetic

The distribution of blood group substances A, B, H, Le‐a and Le‐b in normal and neoplastic salivary gland tissue was evaluated by means of immunohistochemistry. The serological ABH blood group status of one third of the patients was known. Lewis blood group and secretory status were not known. In normal tissue, expression of blood group antigens corresponded to the serological blood group. Blood group substance H was present in almost every gland, regardless of the serological blood group. In submandibular glands, Le‐b was rather selective for mucous acini. In tumors, a relationship of blood group expression to a glandular pattern and a high differentiation could be observed. Blood group substances were expressed at a high level in benign and highly differentiated malignant tumors. In poorly differentiated malignant tumors, they were mostly absent. The findings of a retrospective study of 98 minor salivary neoplasms are reported. The patients' ages ranged from 13–79 years and there was an equal sex distribution. Sixty‐one of the lesions were benign, 53 being pleomorphic adenomas and 8 monomorphic adenomas. Of the malignant tumors, 19 were mucoepidermoid tumors, 12 adenoid cystic carcinomas, 4 adenocarcinomas, 1 carcinoma ex‐pleomorphic adenoma and 1 epidermoid carcinoma. One striking finding was the difference in age at the time of presentation for patients with muco‐epidermoid tumors compared with those with adenoid cystic carcinomas. The effect of human and murine gamma interferon (IFN‐γ) on Class II (DR and DQ) expression by human gingival Langerhans cells (LC) was examined using an organ culture system. Human and murine IFN‐γ increased the expression of both DR and DQ antigens during a 72‐h period. Maximal levels of DR expression (100% of T6+cells) were produced within 6 h of IFN‐γ treatment and were maintained during culture. DQ expression was maximal (100% of T6+cells) at 18 h and was more labile than DR. The effect of IFN‐γ on DR and DQ was abrogated by prostaglandin E2 (PGE) while indomethacin, and inhibitor of PGE synthesis, mimicked the effect of IFN‐γ on DR and DQ expression. Gingival keratinocytes maintained their expression of DR for 72 h in the presence of γ‐IFN, but did not express DQ under any of the culture conditions used in this study. These results demonstrate that γ‐IFN induces differential expression of Class II antigens in the oral mucosain vitro, and suggest that γ‐IFN and PGE may be involved in the control of DR and DQ

To explore the nature and importance of mononuclear cells of different phenotypes in oral premalignant lesions and oral cancer, we studied biopsy specimens from 21 oral red and/or white lesions (6 hyperkeratosis, 3 mild dysplasia, 4 severe dysplasia and 8 squamous cell carcinoma), using monoclonal antibodies and avidin‐biotin‐peroxidase complex staining. Peripheral blood samples (J'B) from 4 normal subjects and 5 reactive lymph nodes (LN) were used as controls for the technique. T11‐positive cells were the predominant phenotype (74–78%) in all cases examined. The T4/T8 ratio in severe dysplasia was significantly lower than that in mild dysplasia (p ≤. 0.05). These observations support the hypothesis of a role for cellular immune responses in oral premalignant lesions and oral cancer. The predominance of T cells may represent the local expression of immunity against antigens (viral or other). The decreased T4/T8 ratio observed in severe dysplasia may represent a transitory stage of local immunosuppression, which may be of critical importance for the progression into carcinoma.
