The incidence of dental caries, and chemical, microbial and electron histochemical analyses of plaque and its acidogenic potential were determined in groups of monkeys fed a cariogenic diet for 3 years in which sucrose was supplemented with phytate or pyridoxine or replaced by invert sugar while the controls received sucrose alone. Only phytate affected a marked reduction in the incidence of caries which did not correlate with changes in plaque composition which suggests that phytate acts by reducing enamel solubility. Pyridoxine had a small but insignificant cariostatic effect; invert sugar was as cariogenic as sucrose.

Dentinal molar fissure caries in Liverpool albino rats, produced by initial exposure to dietary sucrose, could be significantly reversed by subsequent exposure to a xylitol-supplemented starch diet (X-S St). An unsupplemented starch diet had no significant effect on caries under the same conditions. Alternate feeding of sucrose and X-S St produced a sequential progression and regression of caries, respectively. Continued exposure to X-S St produced a successive regression in caries. The regression in caries can be explained in terms of the increased rate of salivary flow and the attendent rise in pH and calcium concentration that occur when xylitol is introduced into the mouth. Both of these changes should encourage re-mineralisation of a reversible lesion below a temporarily non-acidogenic plaque. Under these conditions xylitol is not merely a bland, non-cariogenic agent but is genuinely exerting a therapeutic action against caries.

Addition of 1–2 mM sodium fluoride (NaF) to complex growth medium caused a pronounced decrease in the incorporation of 3H-glycerol into cellular and extracellular lipoteichoic acid (LTA) by Streptococcus mutans, strain BHT. Cell yield and the final pH of the culture were not significantly affected by these levels of NaF. Similarly, the extracellular concentrations of polymer-derived glycerol were not affected. Radioactivity incorporated into cells from a 14C-protein hydrolysate was not influenced by NaF even at a concentration of 10 mM NaF where the cell yield was decreased by 43%. The results suggest that low concentrations of fluoride inhibit incorporation of exogenously added glycerol into LTA of S. mutans, strain BH

An intraoral cariogenicity test (ICT) was used to study the enhancement of fluoride treatments (2% NaF or 8% SnF2) that resulted from prior cariogenic challenge to human enamel surfaces. Measurements of enamel microhardness and microradiography indicated higher resistance to the cariogenically primed than for the unprimed surfaces. These results are interpreted as an adaptation of enamel to a challenging local environment by the formation of highly caries resistant minerals in the microspaces created by the priming challenge. With this adaptive reaction, tooth surfaces at risk were converted to surfaces of higher resistance than nonprimed surfaces. It is suggested that the concept of enamel adaptation to the cariogenic challenge can be applied clinically to arrest incipient caries, especially in patients who respond well to preventive disciplines that include the use of fluoride treatments.

In this study fluoride was added to an artificial caries system in vitro. Both the fluoride concentration and the pH of the gel system had an effect on subsequent lesion formation. At pH 4.0 the addition of fluoride to a concentration of 10 ppm did not inhibit lesion formation. Gross loss of surface tissue was noted beneath which laminated lesions were formed with histological features characteristic of remineralization deep within the lesion. However, at pH 4.5 the presence of fluoride at the same concentration (10 ppm) resulted in either partial or complete inhibition of lesion formation.

A single application of APF to sound enamel before exposure to an artifical caries system in vitro had no effect on subsequent lesion formation. However, when initial lesions were treated with APF (1.23%F) and re-exposed to the artificial caries system histological changes were produced indicative of remineralization within the lesion. This suggests that topically applied fluoride may exert its maximal effect on initial lesions rather than on sound enamel.

The rate of fluorine uptake from sodium monofluorophosphate by four layers of sound bovine enamel of monofluorophosphate enamel was determined.The rate differed depending on the concentration in the treatment solution and the depth of enamel analyzed.When the enamel was treated with monofluorophosphate concentrations (10 ppm F) the rate was directly proportional to time at all depths analyzed .With higher concentrations (100, 1,000 ppm F) the rate was directly proportional to time only in deep enamel and then only after the first hour.
