The imaging effects of high-density scatter from different metal compositions of plate and screw fixation systems were assessed in cadaveric craniomaxillofacial skeletons with computed tomographic scanning. Differences in image clarity of three-dimensional reconstructions were observed between titanium, stainless steel, and cobalt-chromium-molybdenum alloy onlay fixation systems, which were not significantly altered with or without an overlying soft-tissue density (facial drape). These differences were reduced in a plaster model, in which the plates were in an intrabony location.

A temporal contour deformity is characterized by a concavity or depression in the soft-tissue contour of the temporal region and is associated with exaggerated relief of the lateral orbital rim and the zygomatic arch. The etiology of the deformity is varied, comprising any condition that results in displacement, atrophy, or absence of the temporalis muscle or the superficial temporal fat pad. We describe reconstruction of this deformity with porous high-density polyethylene implants in 16 consecutive patients, treated between July 1988 and September 1990. The etiology of the deformity and the surgical treatment are described. The results of treatment are assessed on long-term follow-up, ranging from 2 to 4 years postoperatively.

Reconstructive surgeons have employed various procedures using either autogenous or alloplastic materials to repair cranial defects secondary to trauma, extirpative surgery, or congenital anomalies. Currently, the choice appears to be dependent on the personal choice or background of the operating surgeon. For years, our preference has been to use calvarial bone grafts as our primary source of reconstructive material. Disadvantages include uneven resorption of the bone grafts and limited quantities. For these reasons, bony substitutes present new possibilities for reconstruction of craniomaxillofacial defects. We evaluated Bio-Oss, which is a natural bone mineral derived from a bovine source that is chemically and physically identical to human bone, as a possible replacement material to reconstruct skull defects in a rabbit model.

We describe an infant with combined sagittal and unilateral coronal synostosis who underwent “total vault” craniectomy for skull reshaping. The operative procedure was interrupted without replacement of the calvarial bone grafts. Follow-up over the ensuing 2 months revealed regeneration of the entire cranium and supraorbital rims, as well as (in contrast to earlier reports) redevelopment of fusion within the suture at the same site noted in the initial operation, associated with similar skull deformity. These observations are reviewed with special emphasis on the theoretical implications for the etiology of craniosynostosis and skull deformity.

Treatment of edentulous mandibular fractures has remained somewhat controversial. A recent review of the literature showed that a number of techniques have been proposed with varying degrees of success. It was my clinical impression that I encountered an extremely high proportion of complications when caring for these injuries. For this reason, I performed a retrospective chart review of all patients with a diagnosis of mandibular fractures admitted to the University of California, Davis, Medical Center during the period July 1, 1989 to June 30, 1991. A total of 11 patients of 349 admissions were found to have edentulous mandibular fractures (3%). They were treated with a variety of techniques, including splints or dentures, intraosseous wire fixation, or rigid internal fixation with miniplates and screws. Successful results were obtained with each technique; however, the overall complication rate was 18% (2 patients), which was consistent with my overall results for the treatment of mandibular fractures. I recommend that no one technique is applicable to each fracture and that treatment must be individualized.

A patient with severe orbital facial trauma with complete dislocation of the globe into the maxillary antrum is presented. Reconstruction of the defect with autogenous bone restored globe position and maintained the visual and ocular-rotary function of the eye.

The healing fracture, autograft, and allograft repair are associated with a panoply of unique biochemical and cellular processes. Temporal harmony between the components of these processes can ensure restoration of form and function to bony deficiencies incurred as a consequence of developmental deformity, traumatic incident, or surgical resection. Part I of this editorial describes the chronobiology of the stages of several types of naturally occurring bone repair materials and the interaction of the component cells and their biochemical products. The emphasis is on growth and inductive factors, which are keys to promoting bone regeneration.
