It is only in the last decade that elbow arthroscopy had developed as an alternative method of diagnosing and treating pathology of the elbow. When performed with careful technique and a good understanding of the bony and neurovascular anatomy around the elbow, arthroscopy can benefit a large majority of patients with a very low complication rate. The few series that have been reported have shown significant improvement in 73% to 94% of the patients treated. In most cases, functional recovery is quite rapid in the postoperative period.

Arthroscopic methods in the knee are used for diagnostic procedures and offer the surgeon a minimally invasive method for repairing a wide variety of injuries in both the acutely and chronically injured knee. These procedures are most often associated with minimal morbidity and early initiation of rehabilitation, and patients may return to work and their daily living activities at a faster rate than with conventional surgical methods. In the acute knee injury, arthroscopy has proven to be a valuable tool in defining the type and extent of the anterior cruciate ligament injury as well as providing definitive assessment of meniscal pathology. Partial menisectomy is the most commonly performed arthroscopic procedure in the knee, acounting for approximately 41% of the total arthroscopic procedures performed. Rapid advancement of arthroscopic techniques in chronic knee injuries, including patellofemoral disorders, osteochondritis dissecans, arthrofibrosis of the knee, degenerative arthritis and cruciate ligament injuries, has resulted in less surgical morbidity without disrupting uninvolved anatomy.

Oxygen uptake (Vo2) was measured and economy calculated during running on a treadmill and during cross‐country running in 14 male and 9 female orienteers using a telemetric system (K‐2, Cosmed, Italy). The cross‐country route comprised 3 parts: horizontal path running, horizontal running in light terrain and running in heavy terrain with obstacles and steep hills. Each subject accomplished the whole test route at a speed corresponding to 96±5% of maximal speed and at maximal speed. The running time at maximal speed was 19±3 min. The running economy was similar in treadmill and path running (211 ± 10 vs 210± 14 ml · kg−1· km−1), but Vo2per km increased in light and heavy terrain to 290 ± 16 and 362±18 ml · kg−1· km−1, respectively. There was no difference in running economy between men and women. The elite group (5 men and 3 women) demonstrated 5% better running economy than the sub‐elite (9 men and 6 women) during running in light and heavy terrain, whereas no difference was seen during treadmill and path running. The aim of the study was to investigate whether results from lactate threshold tests on a treadmill would be influenced by collecting blood during 30‐s intervals (INT) as compared to sampling during continuous running (CONT). Ten well‐trained middle‐ and long‐distance runners ran the two protocols randomly on separate days, with the same speed both times. Values of blood lactate, heart rate and ratings of perceived exertion (RPE) were compared. The results showed no significant difference in any of the variables comparing the two regimens. During CONT and INT, running speed at 4.0 mmol · l−1was 18.6 (±1.2) and 18.7 (±1.2) km · h−1, HR 182 (±10.7) and 183 (±8.1) beats · min−1, respectively. Also at the 2 and 3 mmol · l−1level there were no significant differences in running speed or heart rate. RPE values of legs and breathing at the final speed, just above the 4 mM level, showed no significant difference. The purpose of this study was to examine energy metabolism during two simulated races (skating and classical) in cross‐country skiing. In each race 15 elite subjects skied a 2.75‐km track 5 times. In laps 2 and 4, the skiers were told to skate without poles in the skating race and without the diagonal stride in the classical race. The total exercise time was between 42 and 50 min in the 2 races. The oxygen uptake was measured on flat and uphill terrain during each lap and blood lactate after each lap. The relative mean oxygen uptake for classical skiing was 88% (82‐96) for level and 93% (87‐97) for uphill terrain. The respective means for skating were 91% (85‐96) for level and 91% (81‐97) for uphill terrain. The mean values and range for blood lactate at race speeds were 10.6 mM (7.1‐18.1) and 9.2 mM (4.8‐18.8) for skating and classical, respectively. A relative steady state was achieved after the first lap, although a slight but significant blood lactate accumulation took place until finish (0.04‐0.06 mM min−1). The respiratory exchange ratio in both skiing styles varied between 0.88‐0.90 and 0.92‐0.93 for flat and uphill terrain, respectively, indicating a large lipid oxidation at these very hi

In 18 javelin throwers from all parts of Sweden, referred to our Section of Sports Medicine because of medial elbow pain, an arthrography was performed to study intra‐articular changes. In 11 of the throwers the pain occurred suddenly at the moment a throw was being executed and in 7 there was a gradual increase of pain during training and competition. In the clinical examination 1 week to 72 months after the onset of the symptoms there was a tenderness over the medial collateral ligament in all subjects. No elbow was unstable. On the plain radiographic examination a small calcification was seen at the site of the ligament in 3 cases. The arthrography revealed a leakage of contrast into the soft tissue at the site of the ligament in 10 elbows, and a local synovitis was seen in 16 elbows. We examined 20 consecutively admitted athletes suffering from chronic shoulder pain for more than 6 months following a single shoulder trauma without dislocation. Two of them suffered from a posterior detachment of the glenoid labrum. Both had pain during loading, especially during over‐the‐head activities, with a clicking sensation. No previous dislocation of the involved dominant arm was recorded. Both were resistant to conservative treatment. On examination both patients had slightly decreased range of motion, a positive posterior apprehension sign and a negative impingement sign/test. One shoulder was clinically unstable. Special X‐ray identified one minor avulsion of the posterior margin of the glenoid cavity. Magnetic resonance imaging revealed a total posterior glenolabral detachment in both patients and also a bony avulsion of the glenoid posterior margin in one. Arthroscopy identified the posterior labral lesions and reinsertion of the detached labrum using the Mitek‐anchor system was performed. After 1 year, both were able to participate in shoulder‐demanding sports. Total rupture of the femoral biceps muscle is a rare diagnosis, probably because it is disregarded. Few cases have been reported in the literature, and in all cases surgery has been performed. The 2 patients described both had a complete rupture of the femoral biceps tendon a few centimetres proximally to its insertion at caput fibulae. Both sustained the injury during a soccer game. One patient was treated conservatively and the other operated. The final results were similar, but the conservatively treated patient was fully restored after 2 to 3 weeks, whereas the operated patient took more than 3 months to obtain full function.
