In view of recent reports of serious maternal mortality and morbidity associated with obstetric epidural analgesia, a questionnaire was developed to examine the nature of practice in obstetric anesthesia services. Among the data elicited were the medical responsibility for epidural provision and subsequent care, details of patient monitoring, methods of recognition and treatment of complications, training of midwives including cardiopulmonary resuscitation, availability and quality of resuscitation equipment and drugs, and record keeping and patient consent.

On July 10, 1976, an uncontrollable exothermic process occurred during the production of trichlorophenol in a plant at Meda, 20 km north of Milan, Italy. A cloud of toxic material was released that contaminated approximately 1,800 densely populated hectares, including parts of the towns of Seveso, Meda, Cesano Maderno, and Desio. Not until nine days later was it revealed that the most toxic product released was 2,3,7,8-tetrachloro-dibenzo-p-dioxin (TCDD) and that this substance had been found in various samples collected near the plant.

Over a one-year period, patients with a previous cesarean section were screened in the authors' clinic and instructed regarding the risks and benefits of trial of labor after cesarean section. Patients with a previous lower uterine segment transverse cesarean section or previous lower uterine segment vertical cesarean section, regardless of number, were allowed to undergo a trial of labor, obstetric conditions permitting. Exclusions included a previous classical section, any incision suspected of extending into the upper segment, a prior “T” incision, or a failed trial of labor after a cesarean section previously. Patients were encouraged to come to the hospital early in labor to permit early placement of a fetal scalp electrode and internal pressure catheter. Blood was made available. Oxytocin was used for the usual obstetric indications and epidural anesthesia employed upon request. Uterine ruptures were defined as those requiring surgical intervention and dehiscences were those not requiring intervention and noted postpartum.

The syndrome of persistent fetal circulation, also known as persistent pulmonary hypertension, has been recognized for nearly 20 years. The mortality rate of such infants may approach 50% and management involves very aggressive therapy. The purpose of the current study was to determine possible antepartum and intrapartum risk factors associated with this syndrome and to evaluate their single or combined magnitude of risk.

Both the Centers for Disease Control and the American College of Obstetricians and Gynecologists have recommended that pregnant women in risk groups for AIDS be tested for antibodies to HIV. On the basis of several studies it can be anticipated that obstetricians will be increasingly responsible for patients whose pregnancies are complicated by HIV infection. The current report deals with the antepartum, intrapartum and postpartum management of HIV-complicated pregnancies.

There are few data extant on the seroprevalence of HIV infection in childbearing women. Over an 8 week period (December 1986-January 1987) cord blood samples were obtained at delivery from all children born at Kings County Hospital Center and assayed for HIV by means of ELISA testing and Western blot analysis. Risk factor data for HIV infection were obtained from all newly delivered mothers by interview. The current report was based on data from 602 patients in whom both HIV serology results and interview data were available.
