The first family nurse practitioner program in North Carolina began at the University of North Carolina in Chapel Hill in 1968. This article describes the process of achieving and implementing a framework for the legal accommodation of nurse practitioner practice. The recommendations of a Legislative Study Commission helped to achieve enabling amendments of the Nursing Practice Act and the Medical Practice Act in 1973 which established a Joint Subcommittee of the two groups. The legislation charged the committee with developing rules and regulations for the approval of expanded practice for nurses. Since 1974 more than 150 nurse practitioners have been approved based upon: review of the educational preparation of the nurse; the type of practice setting; descriptions of medical acts to be performed with accompanying protocols; and provision for indirect physician supervision, consultation and emergency back up. The physician, nurse and practice site are approved as a unit. An amendment to the Pharmacy Practice Act in 1974 permits these nurses to be issued a a prescribing number, to write prescriptions for standing orders within their practice protocols and the framework of a general formulary. Controlled substances may not be prescribed. A review of the educational program for acceptance is discussed and future directions for joint subcommittee tasks are projected.

The first 50 graduates of the Pediatric Nurse Practitioner Program of Washington University were surveyed to determine their role perceptions and expectations. Seventy-five percent felt that PNP's should provide well child care, manage minor illness and counsel patients and that PNP's and physicians should jointly manage episodic acute illness and chronic illness. They indicated a preference for well child care and were willing to assume the associated professional responsibilities. They wanted to manage problems without the direct supervision of a physician, but did not want to go into independent private practice. Financially, the PNP's felt they should earn more than non-PNP nurses but less than pediatricians. The graduates felt that their new role was unique in the health care system and should be recognized by the certification and accreditation processes. Eighty-four percent of the PNP's indicated they were satisfied with their jobs.

State Nurse Practice Acts serve as a condition for licensure and professional regulation. As such, many acts recently have been changed to legally accommodate the expanded role functions of nurses in primary care. To assess these changes, state boards of nursing were surveyed regarding the status of their laws. The following state-by-state report describes the results of this survey with additional relevant information regarding the laws as they affect the nurse practitioner.

In 1974, a community based project composed of concerned professionals and lay persons contacted us to design and demonstrate the effectiveness of nursing-family assessment procedures with a small number of families in which disturbances had been identified. While we recognize the limits imposed on our findings by the paucity of our sample size, the abuse-neglect families with whom we worked carried a distinguishing set of characteristics which, because of their consistancy, seemed important to report. In addition we wished to share with other health professionals the systematic approach to assessment and intervention which we found useful in establishing rapport and effecting change.

The patient's history and physical findings can be supplemented radiologically. The film shown reveals osteoarthritis of the left hip. This degenerative process has become apparent during middle age secondary to trauma at age 18. The systemic review of the film provides the viewer with valuable additional data as well.
