Active in the United States for the past 25 years, the women's health movement was originally an outgrowth of the larger feminist movement and shares many of the same assumptions. The women's health movement has been successful in increasing public awareness of the problems Involved In the delivery of health care to women and effecting changes In that health care.This article seeks to identify societal contributions and specific events that resulted in the occurrence of this social reform movement, enumerate some of the accomplishments, and suggest why health care providers would benefit by understanding this phenomenon.

Laparoscopy is considered one of the first truly consumer-driven medical advances. It is estimated that 30% to 70% of all gynecologic surgery currently performed via laparotomy incision could be completed by using the laparoscope. The emerging trend in recent years is the change from using the laparoscope in gynecology for diagnosis and tubal sterilization to treating ectopic pregnancy and ovarian neoplasm. The 1991 and 1992 Gynecologic and Obstetrical Society's review and recommendations of this surgical specialty reflect a 1.5-fold increase in the number of laparoscopic procedures reported in 1988. Pelvic pain is the most frequent Indication for operative laparoscopy. There is a need for further evaluation of laparoscopy, including an evaluation of cost-savings claims and an evaluation of necessary recovery time in comparison with laparotomy.Primary care providers can effectively monitor patients' potential surgical needs and appropriately counsel patients when armed with knowledge of the benefits and risks of operative laparoscopy. This article provides a review of the history, purpose, benefits, risks, and patient surgical outcomes of operative laparoscopy.

The increased Incidence of violence in our society has led to a concern for personal safety. For self-protection, many individuals are turning to personal aerosol protection devices (PAPDs), which are readily accessible to the general public. The greater use and misuse of these noxious chemicals has led to an increase in injuries associated with exposure. Health care providers need to be aware of the modes of action of PAPDs, the presenting exposure symptoms, first aid measures, and decontamination procedures of the environment and the victim's belongings. This article provides a thorough discussion of PAPDs and will assist the clinician in delivering primary care and health education to victims of PAPD exposures.

A fever is defined as a rectal temp over 100.4° F. Fever occurs when the hypothalamic thermoregulation center resets the temperature set point in response to a chain of events initiated by the inflammatory response. Glass thermometers remain the gold standard and electronic thermometers are generally acceptable, but studies do not consistently support the use of Infrared ear thermometers in children under 3. Evaluation of the sick child Includes observation, assessment of age and temperature risk factors, history and physical, and lab tests. To aid in the assessment of how ill or “toxic” a child appears, the Yale Observation Scale is used. Acetaminophen remains the antipyretic of choice. Febrile seizures are generally benign. Tepid sponge baths are only slightly more effective than acetaminophen alone in reducing fevers, but may be useful for children with a history of febrile seizures or liver disease. While there are many causes of pediatric fevers, they can be grouped into three general categories: fever with localizing signs, fever without localizing signs, and fever of unknown origin. Children with a localized infection are treated with antibiotics, antipyretics, and parent education; children with fever of unknown origin are referred for more In-depth evaluation. The management of children presenting with fever without a source is discussed in detail.

The purpose of this descriptive exploratory study is to describe the experience of successful smoking cessation in adult women. The convenience sample included 10 women, ages 25 to 42, who had abstained from smoking for at least 6 months but not longer than 3 years. A semistructured interview format was used to elicit descriptions of the experience of successful smoking cessation from these subjects. The interview format explored the experience, including Initial contemplation, the process of quitting, and maintenance of smoking abstinence. Interviews were audlotaped, transcribed, and then analyzed using methods outlined by Miles and Huberman [1]. Four themes emerged from the data: evolving commitment to health and personal growth, being stigmatized, changing conceptualization of smoking, and smoking cessation as a relational phenomenon. These findings were consistent with Pender's Health Promotion Model and have implications for nurse practitioners who counsel women on smoking cessation.
