Myofascial pain syndrome (MPS) is one of the least understood yet commonly encountered problems In the outpatient setting. Myofascial pain syndrome is a painful disorder characterized by trigger points (TrPs), which are hyperirritable spots causing referred pain. Myofascial pain syndrome is frustrating to patients and clinicians. Unfortunately, MPS often goes unrecognized, misdiagnosed, or mistreated, leading to unnecessary pain, suffering, and disability. When treated properly, MPS has an excellent prognosis. Besides specific TrP therapy, treatment involves lifestyle changes and long-term management to prevent recurrences. A multidisciplinary approach utilizing primary care providers, physical therapists, occupational therapists, and other health care professionals is essential to correctly assess and treat MPS. Primary care providers knowledgeable about MPS are in a pivotal position to identify this disorder and to intervene appropriately.

Health care providers who supply primary care to women will at some time encounter a patient presenting with symptoms of abnormal uterine bleeding and secondary dysmenorrhea. Adenomyosis, an elusive uterine disease, is a frequently ignored causative factor of these symptoms. Adenomyosis is considered a benign uterine condition in which the endometrial glands embed into the myometrium of the uterus. It is usually found in multiparous, middle-aged women who present with heavy, painful, cyclical bleeding associated with menstruation. This article addresses the incidence, etiology, symptomatology, and treatment modalities for adenomyosis. Implications for health care providers are discussed.

Toxic synovitis, an acute inflammatory condition of the hip, is the most common cause of limp and acute pain of the hip in children under 10 years of age. Usually, the synovitis and joint effusion are present unilaterally. The etiology of the condition is unknown, although in a few cases a recent history of an upper respiratory tract infection may be present. The child with toxic synovitis may complain of a limp and pain in the hip, the anteromedial aspect of the thigh, and the knee. The white cell count and erythrocyte sedimentation rate may be slightly elevated, as is the body temperature. Ultrasound is recommended as the primary imaging tool in the diagnosis and treatment of toxic synovitis. Septic arthritis, Perthes disease, and osteomyelitis are a few of the differential diagnoses that the practitioner should consider. Most cases can be managed with bed rest at home and administration of a nonsteroidal anti-inflammatory medication. Follow-up care should occur 2 weeks after diagnosis to ensure there is no recurrence of the joint effusion or progression to avascular necrosis. Radiographs of the hip should be repeated at 1 month and 3 months to complete the patient follow-up.

Nursing leadership is essential to manage change; enhance linkages with other health professionals, the public, and the political system; and conduct research regarding clinical practice and system effectiveness and efficiency. The change that is needed and under way in the United States is tri-dimensional (health care reform, workforce reform, and curriculum reform). All three actions must occur in order to create an affordable, accessible, and user-friendly system.During this change, the challenges that are most salient for nurse practitioner faculty are to: differentiate roles among physicians, nurse practitioners, and other nurses; decrease costs while maintaining quality of care; create a supportive environment for advanced practice nurses, with consideration given to barriers to practice, and collaborative and Independent practice; and maintain integrity in the education of both acute care and primary care nurse practitioners. It is imperative that the nursing profession maintain unity throughout these changes and promote initiatives that will ensure accessibility, quality, and affordability of health care, as well as enhance the relevance of education and practice.
