Women who have undergone implantation of a breast prosthesis have been exposed to a variety of health risks, many of which were unknown to them at the time of surgery. Recognition of health hazards has prompted the Food and Drug Administration to restrict implantation of these devices while study of safety and efficacy are undertaken. Capsule contracture, or the contraction of the fibrous tissue surrounding the prosthesis, is the most frequently occurring complication. Exposure to an implant does not appear to heighten the risk for breast cancer, but the opacity of a prosthesis substantially reduces the diagnostic capability of mammography. Silicone, used in the manufacture of implants, is linked to immunologic changes that result in connective tissue disorders. Most often this condition is scleroderma. Damaging effects from toluenediamine, a disintegration product of the polyurethane coating, have been reported. Whether the concentration of this chemical reaches toxic levels in women with prostheses remains to be clarified by current investigations.

Prostatitis, the most common urologic disease in men, afflicts between 25% and 50% of all adult men. It is a complex and not well understood condition, partially because of Its often obscure pathogenesis. There are four prostatitis diagnostic and treatment categories: 1) acute bacterial prostatitis; 2) chronic bacterial prostatitis; 3) nonbacterial prostatitis; and 4) prostatodynia. This article reviews prostatic anatomy and pathogenesis, as well as diagnostic and treatment categories. This information can assist the primary care practitioner in the identification and management of this common and often puzzling condition.

The American Academy of Ophthalmology and the National Eye Institute's national eye health care program's goals are to eliminate preventable blindness as a sequala of diabetes by the year 2000. To successfully implement the program, there needs to be a collaborative effort between primary care providers and ophthalmologists. The various primary health care providers treat approximately 80% of the diabetic population and are therefore in an excellent position to screen patients at risk for vision threatening problems and arrange for timely ophthalmic referral. This article provides an overview of the progression of diabetic retinopathy, the latest research findings and the recommendations for treatment and eye care guidelines set forth by the American Academy of Ophthalmology and the American Diabetes Association. Patient education, early recognition of asymptomatic ocular manifestations, and timely referral and treatment are the keys to prevent blindness.

Research has shown that the experience of losing a child Is by far the most painful. It is documented that bereaved parents have more intense symptomatology than adults grieving other types of loss, such as the loss of a spouse or parent. However, there are fewer studies that focus on parental grief than on other types of grief. This article presents case reports of two mothers grieving the deaths of their children. This presentation is unique because grief as a process is explicated from the perspective of the mothers by means of qualitative and quantitative analysis. Taped semistructured interview sessions were conducted monthly and analyzed using grounded theory. These results were compared to results obtained from the analysis of two quantitative symptom inventories—the SCL-90-R, a psychological distress scale, and the GEI, a 135 item grief experience inventory. The experience of maternal grief that is presented through case examples will enable practitioners to connect theory with practical application to ease the suffering of grieving parents.
