Parenteral substance abuse is widespread. Although treatment for opiate dependence and cocaine abuse takes place in specialized programs, abusers frequently present in primary care settings for management of associated health problems. This article reviews 1) the two drugs that are most commonly abused parenterally (heroin and cocaine); 2) treatment options, including methadone maintenance and narcotic antagonists; and 3) the primary care and health maintenance issues of this population. Psychological aspects of caring for these patients are discussed. Physical findings and diagnosis and management of the complications of parenteral drug abuse are presented.

Although hardly a recent phenomenon, the battered woman has still not been a clearly recognizable presence to primary caregivers. Unawareness of actual victims stems from an incomplete understanding of the reality of the “battered wife syndrome,” lack of knowledge about the various forms of abuse and the silence of the victims themselves. The victim rarely reports abuse to the primary care provider without being asked. She does, however. present with common psychosomatic complaints, such as choking, gastrointestinal disorders or nervousness. Her injuries may range from bruises to fractures, and the explanation of these injuries is usually inappropriate. Characteristically, the victim of abuse has low self-esteem and anger that has been internalized. These lead to self-destructive behaviors such as alcohol or drug abuse. Many of the victim's behaviors are predictable and best understood in the contexts of learned helplessness, a cycle of violence or anticipatory fear. Recognizing that child abuse and spouse abuse often occur within the same family, the nurse practitioner can play a vital role in halting the cycle of family violence. Early recognition, knowledge of the forms of legal recourse in the community and intervention by the nurse practitioner will have an impact on the morbidity and mortality of this social problem.

A careful history, including smoking habits, presence of chronic lung disease in the family history and symptoms of cough, dyspnea and wheeze, will often give the first clue to the presence of chronic lung diseases. The assessment of the patient includes a physical examination and simple spirometric measurements of airflow and volume from fully inflated lungs. When symptoms and airflow abnormalities are present, concern for the patient's outcome is appropriately heightened. This will cause the health care provider to decide on additional observations or evaluation to help prevent the consequences of otherwise undetected chronic lung diseases.

In an era where issues concerning cost and quality of care are constantly raised, it will be increasingly important that expanded-role nurses be able to define their unique contributions to primary care. This will require nurses to self-inventory their nursing processes and conduct research that serves to link nursing action processes to health care outcomes. In order to lay the groundwork for accomplishing this goal, this article provides a brief assessment of the strengths and limitations of an existing primary care process measure (the nurse practitioner rating form), and furnishes a conceptual framework for constructing an alternative process measure —the functional message behavior inventory.
