Health care settings and providers are not immune to violent or disruptive behavior by clients or by strangers off the street. Verbal abuse and threats of violence can escalate to physical attacks directed against health care practitioners. This article discusses the management of violent or dangerous patients in the primary care setting. Characteristics of potentially violent patients are reviewed, along with assessment clues useful in predicting violent behavior. Specific interventions to counteract or diffuse a potentially violent situation are proposed.

Tuberculosis and a multidrug resistant form of TB have reemerged in epidemic proportions. Tuberculosis is more prevalent among HIV-Infected individuals, the homeless, foreign-born individuals from countries with high rates of TB, prison inmates, individuals in long-term care facilities, individuals from low-socioeconomic backgrounds, and intravenous drug users. Urgent care centers, which have gained popularity as an easy access to medical care, are at risk for seeing undetected infectious patients. By developing a TB control plan for ambulatory care centers, specific triage criteria can be instituted to promote early identification of the potentially infectious patient and provide guidelines for identification, prevention, environmental controls, education, and follow-up of exposed health care workers. This article discusses the mandatory OSHA policy for occupational exposure to tuberculosis based on the 1993 CDC guidelines for TB transmission and how a TB control plan for ambulatory care centers can be developed from these guidelines.

Lesbians are often “invisible” in primary care settings and literature. They frequently feel anxious, mistreated, and misunderstood by providers if they reveal their sexuality. Increasingly these women are “coming out” to providers and asking for knowledgeable and sensitive care. This article reviews the literature on lesbian health care and makes suggestions about history taking and health promotion that is sensitive to the needs of lesbian clients.

Despite notoriously low immunization levels among children in the United States in recent decades, provisional statistics for 1994 are encouraging. This may be related to several recent governmental initiatives that have focused attention on immunizations. A new vaccination schedule, effective January 1995, marked major progress in the clarification of immunization guidelines. This article discusses current recommendations for both pediatric and adult immunizations. The text includes information about the current incidence of communicable diseases, recent governmental policies affecting availability of vaccines, standards for immunization practice, and guidelines for storage and handling of vaccines. A table outlines recommended vaccines in the following areas: 1) vaccine antigen, dose, and route, 2) routine schedule for administration, 3) precautions and contraindications, 4) adverse reactions, 5) serious adverse reactions, and 6) education.
