ABSTRACTA febrile seizure is a benign convulsion that occurs in infants or small children and is brought on by fever without evidence of meningitis or encephalitis. Little is known about the etiology; however, it is associated with disease processes outside the central nervous system. In most children, the primary causative factors appear to be the height and rapidity of temperature elevation, which usually exceeds 101.8° F (38.8° C). Seizures occur most often during the temperature rise itself rather than after a prolonged elevation. Variation persists in interpretation, evaluation, and treatment of children with febrile seizures, rendering the diagnosis one of exclusion. Pharmacologic treatment for febrile seizures should be easy to administer, have virtually no adverse effects, and be remarkably efficacious. Along with invasive intervention and pharmacologic management, the clinician must also provide counseling during this stressful event.

ABSTRACTVaginitis resulting from bacterial, fungal, or protozoal infections can be associated with altered vaginal discharge, odor, pruritus, vulvovaginal irritation, dysuria, or dyspareunia, depending on the type of infection. Bacterial vaginosis, which is primarily characterized by a malodorous discharge, is common in women with multiple sex partners and is caused by the overgrowth of several facultative and anaerobic bacterial species. Vulvovaginal candidiasis is characterized by pruritus and a cottage cheese‐like discharge. Vaginal trichomoniasis, a sexually transmitted disease caused by an anaerobic protozoan parasite, is associated with a copious yellow or green, sometimes frothy, discharge. Differential diagnosis of these infections requires a thorough history, vulvovaginal examination, and simple laboratory tests, including microscopy of the vaginal discharge. The information obtained from this workup should enable an accurate diagnosis. Topical or oral metronidazole is the treatment of choice for bacterial vaginosis; terconazole, or other antifungals, for vulvovaginal candidiasis; and oral metronidazole for trichomoniasis.

ABSTRACTSimple traumatic lacerations are one of the most common reasons for visits to the emergency department. Traditionally, wound repair has been performed using sutures. The use of tissue adhesives is an approved and popular alternative method of wound closure in several countries. Numerous published studies have found that, with appropriate use, repair of lacerations using tissue adhesives is faster, less painful, and more economical than suturing. Tissue adhesives result in fewer wound infections than suturing, and cosmetic outcomes have been comparable to or better than suturing. The newest product, 2‐octylcyanoacrylate, received Food and Drug Administration approval in 1998 and is being marketed in the United States. The technique of wound repair with tissue adhesives is easily mastered and is a useful skill in the management of simple lacerations. This article provides a comprehensive review of the literature on the use of tissue adhesives and discusses them as an alternative to traditional methods of wound repair.
