Arthritis is one of the most common chronic illnesses managed in primary care. Osteoarthritis and rheumatoid arthritis are two common types the provider must distinguish between in terms of diagnosis and treatment. Osteoarthritis, the most common form, typically occurs in people more than 60 years of age and involves cartilage destruction. Signs and symptoms are local and include cool, bony joints and arthralgia that worsens with weight bearing. Treatment includes acetaminophen, nonsteroidal anti-inflammatory drugs (NSAIDs), exercise, and joint arthroplasty in severe cases. Rheumatoid arthritis is a systemic disease that results in symmetrical joint inflammation along with constitutional symptoms such as fatigue and depression. Current treatment recommendations include early use of disease modifying anti-rheumatic drugs along with NSAIDs. The key to arthritis management is early diagnosis and treatment to prevent further joint destruction and maximize functional ability.

The adult patient with first-time seizures often presents to the primary care and urgent care settings. It is estimated that 1% of the population of the United States has a diagnosed seizure disorder, with more than 125,000 new cases identified each year. Because of the complexity of assessment and intervention required for the patient with seizures, it behooves the health care practitioner to be aware of diagnostic criteria, current management techniques, and evaluation of treatment protocols. This article discusses various types of seizures, offers information regarding a differential diagnosis, and addresses the implementation of pharmacologic therapies as well as ongoing care of the patient.

Primary care providers are recognizing the need to be able to establish protocols for children with complex conditions, such as spina bifida. Best practices are to form partnerships with families to address the complex health care needs for their children who require comprehensive care, coordination of community services and resources, regular communication with interdisciplinary and specialty teams, and preparation for transition into young adulthood. Health status and quality of life for children with spina bifida depends in part on preventing complications or further disability and maximizing functional independence and inclusion in society. The content in this article presents special considerations through guidelines for medical and psychosoclal aspects of primary health care for children with spina bifida.

Toxoplasma gondiiis a frequent cause of subclinical latent human infection and an important opportunistic pathogen that may cause severe disease in Immunocompromised patients. Patients with AIDS who have antibodies toT. gondiishould be considered at high risk for development of clinical disease (toxoplasmosis). Reactivation of latent infection in the central nervous system is a common HIV/AIDS-related complication in these patients. Typical presenting symptoms are headache, confusion, fever, and focal neurologic deficits. Routine serologic tests cannot distinguish active from latent infection. Neuroradiologic studies may be highly suggestive of toxoplasmic encephalitis, but the definitive diagnosis can be made only by demonstration of toxoplasma in brain tissue. The unique pathogenesis of toxoplasmic encephalitis in patients with AIDS necessitates intensive primary therapy followed by lifelong suppressive therapy. Clinical and radiographic improvement Is usually rapid with appropriate treatment.

The enormous and rapid changes taking place in health care delivery are forcing institutions, organizations, and individual providers of care to reexamine their relationships and create new methods of assuring both quality of care and safety for consumers. One result has been the emphasis on systematic methods of credentialing, privileging, and measuring and evaluating an individual's qualifications and actual performance. This article discusses the credentialing and privileging processes that are either in place or evolving for physicians in the managed care health care systems. With the continued evolution of managed care in this country, advanced practice nurses may need to expand their credentialing process. This article provides information on the evolutionary events leading to credentialing and privileging, current and anticipated credentialing measures, sources of measurements, medicolegal issues, and suggestions for expansion of nurse credentialing. A comprehensive credentialing system will provide additional evidence of the nursing profession's capacity to play a major role in the reformed health care system.
