摘要:
Progress In genetics has endless implications for primary care and for people at risk for hereditary breast cancer. Nurse practitioners will be increasingly challenged to order and interpret genetic tests, provide genetic counseling, and assume responsibility for protecting patient privacy. Yet primary care practitioners may not be completely prepared to provide the necessary background information. This article provides guidelines for evaluating family history risk for Inherited breast cancer risk. Also discussed are implications for presymptomatic testing of the BRCA1 and BRCA2 genes for adult-onset breast and ovarian cancer.

摘要:
Meningococcal disease is an Infection caused byNeisseria meningitidis, a gram-negative diplococcus that Is the leading cause of bacterial meningitis in children and young adults in the United States, with an estimated 2,600 cases reported each year.N. meningitidisinfection rates are highest In children 3 to 12 months of age. Four distinct clinical situations are associated with meningococcal infection. The most common is asymptomatic nasopharyngeal colonization. Benign bacteremia is discovered in the absence of classical clinical findings of meningococcemia, but blood cultures are positive forN. meningitidis.Meningitis, the most common pathologic presentation, is associated with fever, headache, and nuchal rigidity. The mortality rate is about 5% in children and 10% to 15% in adults. Meningococcemla, the most severe form of infection, may involve petechial rash, hypotension, and disseminated Intravascular coagulation. It is a fulminant condition that can, if untreated, progress from Initial symptoms to coma and death in 12 to 48 hours. Spread of these endemic cases can be controlled by administering prophylactic antibiotics to close contacts of patients.

摘要:
Smoking cigarettes remains the leading cause of preventable illnesses and premature deaths In the United States. Although approximately half of living Americans who have ever smoked have quit, recent studies show that more than 70% of current smokers would like to stop smoking. Health care providers can enhance smoking cessation In their clients by performing assessments of both the physical and psychologic addictive aspects of smoking as well as past and current cessation barriers. Based on the assessment, the clinician should formulate a treatment plan by individualizing appropriate education, counseling, motivation, and pharmacologic support. This article reviews the prevalence and hazards of smoking and supplies health care providers with interventions to assist clients in becoming and remaining nonsmokers.
