On September 8, 1997 (day 0), an express train in Aquitaine, France, crashed into a petrol tanker, which immediately burst into flames and set the first carriage of the train on fire. We describe the dental technique and methodology which led (day 2) to the positive identification of 92% of these very heavily carbonized remains. In only 1 case, and only to be totally certain, did the investigators have recourse to use molecular biologic techniques. The use of this specific odontostomatologic technique (craniomandibular disarticulation with bone samples) and excellent collaboration with the Identification Unit for Mass Disasters Victims (CIVC) precluded recourse to any reconstructive identification.

A 34-year-old woman with a history of asthma and oral contraceptive use died suddenly. Autopsy examination showed chronic pulmonary emboli with an acute pulmonary saddle embolus. An underlying congenital thrombophilic disorder was considered. Molecular studies on DNA isolated from blood using polymerase chain reaction (PCR) and restriction fragment length polymorphism (RFLP) analysis revealed coagulation factor V Leiden mutation. The incidence of venous thromboembolism in patients with factor V Leiden mutation and associated activated protein C (APC) resistance is discussed.

Although China has a long history of forensic medicine, with the first standard text published in 1247, modern practices appeared only in the 1930s under Professor Lin Ji. After the founding of the People's Republic of China in 1949, there was a period of rapid development, which was later interrupted by the Cultural Revolution of 1966-1976. Today, China has about 10,000 experts in forensic medicine organized within the separate agencies of police, prosecutor's offices, courts, universities, and the Justice Ministry. Eight medical colleges, the Institute of Forensic Sciences of the Ministry of Justice in Shanghai which publishes theJournal of Forensic Medicine,and the Forensic Medicine Association of China which publishes theChinese Journal of Forensic Medicineare key organizations.

The aim of this study was to investigate the permeation behavior of a large molecule through a venous wall; hemoglobin was chosen as a model substance. In vitro experiments were performed using a Chien-Valia diffusion chamber. Postmortem, hemolyzed, and fresh nonhemolyzed blood samples were investigated as permeants. Vein patches from vena cava inferior and vena jugularis interna were used as diffusion barriers. Applying this technique, extravasation of hemoglobin was detectable. The portion of hemoglobin molecules passing through the vascular wall depended on time, vein type, and graduation of hemolysis. The passage of hemoglobin across the wall of a large vein suggests intravascular changes in drug concentrations from postmortem blood samples not to be restricted on the unbound portion of the particular drug.

Death during autoerotic asphyxial activity is usually considered accidental due to unanticipated failure of the device that was used to induce hypoxia during sexual arousal. Two cases of hanging are reported in which apparently typical features of autoerotic asphyxial activity were present. As a result of the discovery of a suicide note in one case and the absence of certain characteristic features in the other, the deaths could not be designated accidental. Although the majority of deaths that occur during autoerotic asphyxial episodes are accidental, the possibility of suicide must always be entertained, even in cases that may initially appear typical.

We describe 3 unusual autoerotic fatalities in which propane gas was used to induce hypoxia. To our knowledge, these are the first cases of their kind to be reported and described in depth, although brief references to similar cases have appeared. The case histories of 3 white males, aged 22, 42, and 62 years, are described with emphasis on the scene and circumstances and the identification of propane from autopsy materials. In one case, unusual paraphilia and multiple foreign bodies in the rectum were noted. These 3 cases illustrate an unusual form of autoerotic practice in which hypoxia is induced by propane inhalation, unlike the usual neck compression that has been previously described in depth. The importance of scene reconstruction and an awareness of this phenomenon is stressed.

We report two cases of unexpected death due to metastatic gestational choriocarcinoma and provide a review of the literature regarding these neoplasms. Our cases illustrate the extreme importance of medical follow-up of patients after an ectopic pregnancy or therapeutic abortion.

A 14-year-old girl experienced sudden onset of weakness progressing rapidly to paralysis. She died 7 days later from a massive pulmonary thromboembolus. Autopsy revealed extensive infarction of the spinal cord with a fibrocartilaginous embolus of the corresponding segment of the anterior spinal artery.
