摘要:
Reduction in blood flow to the head results from the use of the neck hold commonly referred to as the carotid sleeper. Using ultrasonic and laser-Doppler blood flow monitoring devices, quantitative assessment of the decrease in blood flow to the head was measured. During the application of the carotid sleeper neck hold, blood flow to the head decreased by 85% (mean value) and reached its lowest point in 6 seconds. Other physiological parameters affected by the neck hold are also reported.

摘要:
The sex of unidentified skeletons is not always easily and correctly determined by the general medical examiner with no formal training in forensic anthropology. Sometimes the diagnostic procedure may be extremely complicated when handling fragmented bones. The present preliminary observations concern the evaluation of the size of the foramen magnum in 40 skulls (20 of each sex). Our results indicate that this method may be useful in the sexing of skeletal remains under the above-mentioned conditions as well as when an expert in forensic anthropology or a crime laboratory is not available.

摘要:
Calcification of the annulus fibrosus at the base of the posterior mitral leaflet is a degenerative cardiac lesion commonly seen in the elderly. Coexisting calcification of the anterior mitral leaflet with resultant mitral stenosis is rare. We document a case of severe mitral annulus calcification with involvement of the anterior leaflet and significant mitral stenosis occurring in an 89-year-old woman who presented as a sudden death. The lesion can be readily distinguished macroscopically from mitral stenosis of rheumatic etiology.

We studied the ultrastructure of human skin in a particular form of postmortem transformation: so-called “corification.” Six- and 12-month-old corpses were examined. The epidermis showed a remarkable ultra-structural preservation of its components comparable to freshly fixed material. The dermis was less well preserved with altered bandage of collagen fibrils and amorphous precipitates in the ground substance. No explanation is found for such a phenomenon.

External beveling of entrance wounds may be seen either when a bullet strikes the skull tangentially, producing overlapping entrance and exit wounds (keyhole defect) or, more commonly, in entrance wounds when the bullet strikes the skull perpendicularly to the surface of the bone. In the latter cases such external beveling may be partially or completely circumferential. The mechanism of producing beveling of an entrance wound when a bullet strikes the skull perpendicularly to the surface is not completely understood. Several proposed mechanisms are briefly discussed. The possible misinterpretation of an entrance wound as an exit wound is stressed. Such a misinterpretation should never occur when a complete autopsy is performed.

The physician, like any other human being, is subject to emotional problems, greed, jealousy, and instability of an individual under stress as well as mental disease which may all bring out improper behavior or criminal activity. Over the last two centuries, there have been instances where physicians have committed deliberate acts of murder on wives, relatives, patients, and others. Their motivation has ranged from ridding themselves of a nagging wife or for insurance and inheritance gains to insane acts. The methods used have usually been associated with poisons and drugs, of which the physician may have great familiarity. The frequency of physician-involved murders seems to have increased in the past two decades with several sensational cases being the topics of media coverage and further review in books and visual media, including television and the movies. The investigation of these cases may be very difficult, especially when a drug may be given to a victim which could cause severe problems after small amounts are given. The detection of death as being a murder may be the first problem faced, the demonstration of the fatal agent in the body in the next problem, and probably the most difficult is finding a suspect with the medication in his or her possession or paraphenalia for injecting such as a syringe and needle. The stealth and cunning of the murderer may further complicate the picture for the investigator as well as the reputation of the physician in the community, which is often above reproach so that the public has to be convinced of the actual involvement of the suspect.

摘要:
The recent discovery of a case report in the manuscript collection of the works of Joseph Jones, Professor of Chemistry and Clinical Medicine, Medical Department, University of Louisiana, in the Tulane University library, has brought to our attention another example of the use of scientific expertise in the investigation of a homicide which occurred in 1876 in Donald-sonville, Louisiana. An unusual circumstance in the case provided the clue that tied together the suspect with the murder victim. The use of the microscope and chemical testing by Prof. Jones was unique in that he was able to demonstrate that the blood contained evidence of a malarial infection in the blood; this evidence was found at the scene and on a shirt of the suspect. The victim has been treated for malaria shortly before his death by bludgeoning.

摘要:
An unusual fatal case of an accidental craniocerebral penetrating wound due to a power drill is described. A 25-year-old man fell from a stepladder while perforating a ceiling with a drill to arrange the lights for New Year's Day. During the fall the drill penetrated the right upper jaw, passed through the orbit, the ethmoid and the sphenoid bones, tearing the left internal carotid artery, the left cavernal sinus, and the left base nuclei. Death was due to inhalation of blood and atelectasis. The skin wound was an atypical puncture wound similar to bullet entrance wound.

摘要:
This paper reviews a fatal case of benzphetamine poisoning. The pharmacology of benzphetamine is reviewed with respect to its prototype amphetamine. Methods of detection and quantitation of benzphetamine are summarized. A search of the literature failed to reveal a death from benzphetamine poisoning.
